腹腔镜疝囊高位结扎治疗腹膜透析并发隐匿性腹股沟疝的体会

摘　　要：目的:总结腹腔镜下疝囊高位结扎术治疗腹膜透析并发隐匿性腹股沟疝的临床疗效。方法:回顾分析2012年5月至2019年5月行腹腔镜疝囊高位结扎术治疗的8例腹膜透析并发隐匿性腹股沟疝患者的临床资料,包括一般资料、手术成功率、手术时间、住院时间、术后恢复腹膜透析时间、并发症发生率、复发率等。结果:8例患者平均(45.2±9.5)岁,均为女性,均因终末期肾病行腹膜透析;2例已行血液透析,6例维持腹膜透析。术中应用普理灵3-0聚丙烯不可吸收缝合线行腹腔镜下疝囊高位结扎术,手术时间平均(11.6±5.3)min,平均住院(4.3±1.2)d,术后无明显并发症发生,术后第2天试用腹膜透析,1~2周过渡至术前的透析量与频率。术后随访4~13个月,无复发病例。结论:腹腔镜下疝囊高位结扎术治疗腹膜透析并发隐匿性腹股沟疝具有较好的近期疗效。Objective:To summarize the clinical efficacy of laparoscopic high ligation of the hernia sac in the treatment of peritoneal dialysis patients with occult inguinal hernia.Methods:A retrospective analysis was made on the clinical data of 8 patients with peritoneal dialysis complicated with occult inguinal hernia who underwent laparoscopic high ligation of the hernia sac from May 2012 to May 2019,including general information,surgical success rate,operation time,hospital stay,postoperative resuming peritoneal dialysis time,complication rate and recurrence rate.Results:The average age of 8 patients was(45.2±9.5)years old,all were female;all of them were treated with peritoneal dialysis for end-stage renal disease;2 patients had undergone hemodialysis and 6 patients were maintained with peritoneal dialysis.Laparoscopic high ligation of hernia sac was performed with Prolene 3-0 polypropylene non-absorbable suture during the operation.The operation time was(11.6±5.3)min,and the average hospital stay was(4.3±1.2)d.There were no obvious postoperative complications.Peritoneal dialysis was tried on the second day after the operation,and the dialysis volume and frequency gradually returned to preoperative level during 1-2 weeks.The postoperative follow-up time was 4-13 months without recurrence.Conclusions:The short-term curative effect of laparoscopic high ligation of hernia sac is good in the treatment of peritoneal dialysis patient with occult inguinal hernia.
