摘 要:目的探讨川崎病(KD)患儿血浆可溶性CD100(sCD100)在急性期和恢复期的变化及其意义。方法选取2019年1月至2019年6月确诊为完全型川崎病(CKD)的患儿45例为研究对象,其中发生冠脉损伤者(CAL)者17例,未发生CAL者28例;另选取同时期40例上呼吸道病毒感染儿童为发热对照组,40例健康体检儿童为健康对照组;ELISA法检测KD患儿急性期和恢复期及两对照组儿童血浆sCD100水平;用不同水平人源重组CD100体外刺激人冠脉内皮细胞(HCAEC),ELISA法检测上清液IL-1β和IL-8的水平。结果KD急性期血浆sCD100水平明显高于健康对照组(P<0.05),但与发热对照组比较差异无统计学;发生CAL者血浆sCD100水平高于未发生CAL者(P<0.05);KD恢复期血浆sCD100水平较急性期明显降低,且与健康对照组比较差异无统计学意义;sCD100可刺激HCAEC产生IL-1β和IL-8。结论KD患儿外周血高表达的sCD100可能与川崎病的发生和发展有关;血浆sCD100可能成为新的临床预测CAL发生的监测指标,并具有成为治疗KD新靶点的潜力。Objective To investigate the changes in plasma levels of sCD100 in children with Kawasaki disease(KD)in the acute phase after clinical treatment and its related clinical significances.Methods A total of 45 children diagnosed with complete Kawasaki disease(CKD)from January,2019 to June,2019,including 17 patients with coronary artery lesion(CAL)and 28 patients without CAL,were enrolled as study subjects.40 children with upper respiratory tract virus infection and 40 children from healthy physical examinations were enrolled as febrile controls(FC)and healthy controls(HC),respectively.Plasma levels of sCD100 in all participants were measured using enzyme-linked immunosorbent assay(ELISA).Human coronary artery endothelial cells(HCAEC)were stimulated with recombinant CD100,followed by detecting the levels of IL-1βand IL-8 in the culture supernatants using ELISA.Results Plasma sCD100 levels were significantly increased in acute-phase KD patients as compared to HC(P<0.05),while there were no statistical difference with FC group.Plasma sCD100 levels were substantially higher in acute KD children with CAL than in those without CAL(P<0.05).After effective clinical treatment,KD children in the convalescent phase had a significant reduction in the plasma level of sCD100 which had no significant difference with HC group.Soluble CD100 induced IL-1βand IL-8 secretion of HCAEC.Conclusion High plasma levels of sCD100 may be involved in the development and progression of KD.The plasma level of sCD100 could be used as a new index for predicting CAL and has the potential as a novel KD therapeutic target.
