摘　　要：目的探讨2型糖尿病(type 2 diabetes mellitus,T2DM)肾病患者中性粒细胞/淋巴细胞比值(neutrophil-lymphocyte ratio,NLR)与肾功能水平的相关性。方法回顾性分析我院2017年1月至2018年1月收治在肾内科住院的2型糖尿病肾病患者70例为研究对象,采用单因素及多因素回归分析方法确定影响患者肾小球滤过率值(eGFR)的相关因素。根据ROC曲线结果确定NLR的临界值,并将患者分为NLR低组和高组,比较NLR水平低组、高组与肾功能主要指标的关系。结果单因素分析发现年龄、糖尿病史、HbA1c、Hb、NLR是eGFR的影响因素,多因素分析矫正了年龄、服用降压药等多个因素后显示NLR(P=0.001)、年龄(P=0.036)是影响eGFR的独立因素。NLR的临界值为2.33,NLR值的高低与肾功能指标CysC、BUN、Cr和eGFR密切相关。NLR值与eGFR水平呈负相关(r=-0.313,P=0.008)。结论糖尿病肾病患者肾功能受多个因素影响,NLR值升高是肾功能损伤的危险因素。Objective To investigate the correlation between the ratio of neutrophil lymphocytes and renal function in type 2 diabetes mellitus.Methods A retrospective analysis of 70 patients with type 2 diabetes mellitus admitted to the department of nephrology from January,2017 to January,2018 was conducted.Univariate and multiple factor regression analysis were used to determine the factors affecting glomerular filtration rate(eGFR).According to ROC curve results,the critical value of NLR was determined,and the patients were divided into the low and high NLR groups,and the relationship between low and high NLR levels and the main indexes of renal function was compared.Results Univariate analysis results identified that age,diabetes history,HbA1c,Hb and NLR were the influencing factors of eGFR,while multivariate analysis showed that NLR(P=0.001)and age(P=0.036)were independent factors affecting eGFR after correcting for multiple confounding factors such as age and taking antihypertensive drugs.The critical value of NLR was 2.33,and the NLR value was closely related to the renal function indexes CysC,BUN,Cr and eGFR.The NLR value was negatively correlated with the eGFR level(r=-0.313,P=0.008).Conclusion The renal function of diabetic nephropathy patients is affected by many factors,and the increased NLR value is a risk factor of renal injury.
