血清SAA、HBP联合检测对糖尿病酮症酸中毒合并感染的诊断价值

摘　　要：目的检测糖尿病酮症酸中毒合并感染患者血清淀粉样蛋白A(SAA)、肝素结合蛋白(HBP)水平,评估血清SAA、HBP联合检测对糖尿病酮症酸中毒合并感染的诊断价值。方法选取2017年3月至2019年7月在本院就诊的糖尿病酮症酸中毒患者96例作为研究对象,其中,糖尿病酮症酸中毒合并感染患者(感染组)38例,糖尿病酮症酸中毒未合并感染患者(未感染组)58例。胶体金法检测SAA水平;酶联免疫吸附试验(ELISA)法检测HBP水平;采用受试者工作特征(ROC)曲线评估血清SAA、HBP水平对糖尿病酮症酸中毒合并感染的诊断价值。结果感染组糖尿病酮症酸中毒患者血清SAA、HBP水平较未感染组均明显升高(P<0.05);ROC曲线分析结果显示,血清SAA水平检测的灵敏度、特异性、准确度分别为86.8%、77.6%、81.3%,血清HBP水平检测的灵敏度、特异性、准确度分别为86.8%、72.4%、78.1%,血清SAA、HBP联合检测的灵敏度、特异性、准确度分别为94.7%、94.8%、94.8%;SAA、HBP联合检测诊断的特异性、准确度较单独诊断均明显升高(P<0.05)。结论糖尿病酮症酸中毒患者并发感染后血清SAA、HBP水平明显上调,对糖尿病酮症酸中毒合并感染均有一定诊断价值,两者联合检测诊断价值更高,可用于临床借鉴。Objective To detect the levels of serum amyloid A(SAA)and heparin binding protein(HBP)in diabetic ketoacidosis patients with infection,and to evaluate the diagnostic value of serum SAA combined with HBP in diabetic ketoacidosis with infection.Methods From March,2017 to July,2019,96 diabetic ketoacidosis patients in our hospital were selected as the research objects,including 38 diabetic ketoacidosis patients with infection(the infection group)and 58 diabetic ketoacidosis patients without infection(the un-infection group).SAA level was detected by colloidal gold method;enzyme linked immunosorbent assay(ELISA)was used to detect the level of HBP;the diagnostic values of serum SAA and HBP levels in diabetic ketoacidosis with infection were evaluated by receiver operating characteristic(ROC)curve.Results The levels of SAA and HBP of diabetic ketoacidosis patients in the infected group were significantly higher than those in the un-infection group(P<0.05).ROC curve analysis showed that the sensitivity,specificity and accuracy of SAA were 86.8%,77.6%and 81.3%,respectively,while the sensitivity,specificity and accuracy of serum HBP were 86.8%,72.4%and 78.1%,respectively,and the sensitivity,specificity and accuracy of serum SAA and HBP combination were 94.7%,94.8%and 94.8%,respectively.The specificity and accuracy of combined diagnosis of SAA and HBP were significantly higher than those of single-index diagnosis(P<0.05).Conclusion The serum SAA and HBP levels in patients with diabetic ketoacidosis complicated with infection are significantly up-regulated,which has a certain diagnostic value for diabetic ketoacidosis complicated with infection.The combination of the two markers has a higher diagnostic value and can be used for clinical reference.
