摘  要:目的:分析磁共振成像(MRI)在早期强直性脊柱炎(AS)骶髂关节病变诊断中的应用价值。方法:选取2018年2月-2020年2月于本院住院治疗的60例AS骶髂关节病变患者,分别给予CT以及MRI检查,以手术诊断结果为金标准,比较两种检查方法的异常检出率、影像学表现检出及不同病变分级检出情况。结果:MRI异常检出率为96.67%明显高于CT的80.00%(P<0.05)。MRI与CT对关节软骨肿胀、关节间隙变化、关节面下骨质硬、骨质侵蚀及关节面下骨质囊变检出率比较,差异均无统计学意义(P>0.05)。MRI与CT病变分级检出情况比较,差异均无统计学意义(P>0.05)。结论:MRI应用于AS骶髂关节病变诊断中,异常检出率较高,可明确病变影像学表现、病变分级,为治疗方案的制定提供科学的参考依据,值得借鉴。Objective:To analyze the value of magnetic resonance imaging (MRI) in the diagnosis of early ankylosing spondylitis (AS) sacroiliac joint lesions.Method:A total of 60 patients with AS sacroiliac joint lesions hospitalized in our hospital from February 2018 to February 2020 were selected.All patients were given CT and MRI examinations respectively.The surgical diagnosis results were used as the gold standard to compare the abnormal detection rate,imaging findings and different lesion grades of the two examination methods.Result:The abnormal detection rate of MRI was 96.67%,which was significantly higher than 80.00% of CT (P<0.05).There were no statistically significant differences between MRI and CT in the detection rate of articular cartilage swelling,joint space change,subarticular bone stiffness,bone erosion and subarticular bone cyst change (P>0.05).There was no statistically significant difference between MRI and CT on detection of lesion grading (P>0.05).Conclusion:In the diagnosis of AS sacroiliac joint lesions,MRI has a high detection rate of abnormalities,which can clarify the imaging manifestations and grade of lesions,and provide a scientific basis for the formulation of treatment plans,which is worthy of reference.
