正畸牵引钉与小切口在颧骨复合体骨折治疗中的运用研究

摘　　要：目的:探究正畸牵引钉与小切口在颧骨复合体骨折治疗中的应用效果及炎性应激程度。方法:选取2011年1月-2018年10月期间的86例颧骨复合体骨折患者为研究对象,将其随机分为对照组(头皮冠状入路切开复位内固定术组)和观察组(正畸牵引钉与小切口组),每组各43例。比较两组治疗后的优良率、手术前后的咬合力指标、VAS评分及炎性应激指标。结果:两组治疗后的优良率、手术前后的咬合力指标比较,差异均无统计学意义(P>0.05)。术前两组的VAS评分及炎性应激指标比较,差异均无统计学意义(P>0.05);术后1、3、7 d的VAS评分及炎性应激指标均显著低于对照组,差异均有统计学意义(P<0.05)。结论:正畸牵引钉与小切口在颧骨复合体骨折治疗中的应用效果较好,且可有效控制炎性应激及疼痛程度,在本类骨折患者中的应用价值较高。Objective:To investigate the application effect of orthodontic traction nail and small incision in the treatment of patients with zygomatic complex fracture and inflammatory stress degree.Method:86 patients with zygomatic complex fracture from January 2011 to October 2018 were chosen as the research objects,they were randomly divided into control group(open reduction and internal fixation by coronal approach of scalp group)and observation group (orthodontic traction nail and small incision group) with 43 cases in each group.Then the excellent and good rates after the treatment,bite force indexes,VAS scores and inflammatory stress indexes before and after the operation of two groups were compared.Result:The excellent and good rates after the treatment and bite force indexes of before and after the operation of two groups were compared,there were no statistically significant differences (P>0.05).The VAS scores and inflammatory stress indexes of two groups before the operation were compared,there were no statistically significant differences (P>0.05);the VAS scores and inflammatory stress indexes of observation group at 1st,3rd and 7th day after the operation were significantly lower than those of control group,there were statistically significant differences (P<0.05).Conclusion:The application effect of orthodontic traction nail and small incision in the treatment of patients with zygomatic complex fracture is better,and it can effectively control the inflammatory stress and pain degree,so its application value in these patients with fracture is higher.
