骨折延迟愈合患者血清LAP、GGT、PTH、ALP水平检测的临床意义

摘　　要：目的:探讨骨折延迟愈合患者血清亮氨酸氨基肽酶(LAP)、谷氨酰转肽酶(GGT)、甲状旁腺激素(PTH)、碱性磷酸酶(ALP)、钙(CA)结果检测的临床意义。方法:选取骨折患者102例,根据骨折影像学愈合评分(RUST)将患者分为正常骨性愈合组以及延迟愈合组,同时选取50例健康志愿者作为对照组。于骨折术前以及手术后的第1天、第1周、第1个月时检测并分析血清LAP、GGT、PTH、ALP、CA检测结果。利用重复测量方差分析研究两组骨折患者LAP、GGT、PTH、ALP、CA检测结果在骨折后各个时间点上的差异。结果:同一时间点比较时:骨折愈合组术后1周以及术后1个月的血清LAP检测结果[(41.6±7.3)U/L、(41.7±7.6)U/L]明显低于延迟愈合组术后1周和术后1个月的血清LAP检测结果〔(73.6±8.9)U/L、(96.3±11.8)U/L〕以及对照组的血清LAP结果(37.6±12.3)U/L,差异有统计学意义(P<0.05);骨性愈合组术后1周以及术后1个月的血清GGT检测结果〔(68.2±13.6)U/L、(43.2±10.2)U/L〕与延迟愈合组术后1周及术后1个月的血清GGT结果〔(64.3±11.2)U/L、(48.2±11.3)U/L〕以及对照组的血清GGT结果[(22.3±3.9)U/L]差异无统计学意义(P>0.05);骨性愈合组术后1周的血清ALP、PTH、CA结果〔236.5±20.6)U/L、(49.6±6.8)pg/ml、(2.45±0.10)mmol/L〕明显高于延迟愈合组术后1周的血清ALP、PTH、CA结果〔(189.6±9.8)U/L、(44.8±5.8)pg/ml、(2.24±0.09)mmol/L〕;骨性愈合组术后1个月的血清ALP、PTH、CA结果〔(298.6±22.6)U/L、(52.3±7.9)pg/ml、(2.46±0.13)mmol/L〕明显高于延迟愈合组术后1个月的血清ALP、PTH、CA结果〔(223.6±13.9)U/L、(47.2±7.2)pg/ml、(2.33±0.10)mmol/L〕以及对照组的血清ALP、PTH、CA结果〔(89±8.9)U/L、(42.6±8.2)pg/ml、(2.42±0.07)mmol/L〕,差异有统计学意义(P<0.05)。不同时间点之间GGT结果差异无统计学意义(P>0.05);LAP、PTH、ALP、CA结果差异有统计学意义(P<0.05)。时间和分组间的交互作用对CA、GGT值�Objective To investigate the clinical significance of detection of serum LAP,GGT,PTH,ALP and CA in patients with delayed fracture healing.Method 102 patients with fractures in our hospital were selected and divided into normal healing group and delayed healing group according to the fracture imaging hematology score(RUST)score.50 healthy volunteers were selected as the control group.Detecting and analysing the test result of Serum LAP,GGT,PTH,ALP,CA Before the fracture and on the first day,the first week,and the first month after the operation.Using repeated measures analysis of variance to study the differences in LAP,GGT,PTH,ALP,and CA results between the two groups of fractures at various time points after fracture.Results When comparing at the same time:The serum LAP test results of fracture healing group 1 week and 1 month after operation were(41.6±7.3)U/L and(41.7±7.6)U/L,significantly lower than the delayed healing group(73.6±8.9)U/L,(96.3±11.8)U/L and the serum LAP results of the control group(37.6±12.3)U/L,the difference was statistically significant(P<0.05);Serum GGT test results(68.2±13.6)U/L,(43.2±10.2)U/L in the bone healing group 1 week after surgery and 1 month after surgery,and serum GGT results in the delayed healing group 1 week after surgery and 1 month after surgery(64.3±11.2)U/L,(48.2±11.3)U/L and the serum GGT results of the control group(22.3±3.9)U/L were not statistically significant(P>0.05);The results of serum ALP,PTH and CA in the bone healing group one week after operation were(236.5±20.6)U/L,(49.6±6.8)pg/ml and(2.45±0.10)mmol/L,which was significantly higher than that in the delayed healing group one week after operation〔(189.6±9.8)U/L,(44.8±5.8)pg/ml,(2.24±0.09)mmol/L〕and the Serum ALP,PTH,and CA results in the bone healing group one month after operation were(298.6±22.6)U/L,(52.3±7.9)pg/ml,and(2.46±0.13)mmol/L,which were significantly higher than those in the delayed healing group one month after operation and the results of Serum ALP,PTH,CA(223.6±13.9)U/L,(4
