摘　　要：目的:探讨不同心脏起搏方式对心力衰竭伴心房颤动患者心衰指标、心功能的影响。方法:选择2018年11月-2020年3月于本院进行治疗的78例患者为研究对象。按照随机数字表法分为对照组和观察组,每组39例。观察组采用RVS,对照组采用RVA。观察两组电极导线参数、左心室内收缩同步性及QRS时限、心室起搏、心室感知百分比、心功能、心衰指标变化情况。结果:两组干预前后感知阈值、电极阻抗比较,差异均无统计学意义(P>0.05)。干预后,观察组起搏阈值、心室起搏百分比、Tei指数、LVEDV水平、BNP、CRP水平均低于干预前和对照组,差异均有统计学意义(P<0.05)。干预后,观察组Ts-SD、PPEI-APEI的绝对值、QRS、心室感知百分比、LVEF水平均高于干预前和对照组,差异均有统计学意义(P<0.05)。结论:不同心脏起搏方式对心力衰竭伴心房颤动患者心衰指标、心功能的影响有一定的差异,对比RVA而言,RVS能够更好地改善患者的Ts-SD、PPEI-APEI、QRS水平,改善患者的心室起搏、心室感知百分比,改善机体的心功能以及心衰指标,值得临床推广。Objective:To investigate the effect of different cardiac pacing patterns on heart failure index and heart function of patients with heart failure and atrial fibrillation.Method:A total of 78 patients who were treated in our hospital from November 2018 to March 2020 were selected as study subjects.According to the random number table method,the patients were divided into control group and observation group,39 cases in each group.RVS was used in observation group and RVA was used in control group.The changes of electrode lead parameters,left ventricular systolic synchronicity and QRS duration,ventricular pacing,percentage of ventricular perception,cardiac function,and indexes of heart failure in the two groups were observed.Result:There were no statistically significant differences between the two groups in perceived threshold and electrode impedance before and after intervention(P>0.05).After intervention,the pacing threshold,percentage of ventricular pacing,Tei index,LVEDV level,BNP and CRP levels in the observation group were all lower than those before intervention and the control group,with statistically significant differences(P<0.05).After the intervention,the Ts-SD,absolute value of PPEI-APEI,QRS,percentage of ventricular perception and LVEF levels in the observation group were higher than those before intervention and in the control group,with statistically significant differences(P<0.05).Conclusion:The effects of different cardiac pacing methods on the indexes of heart failure and heart function in patients with heart failure and atrial fibrillation are different.Compared with RVA,RVS can better improve the patients’levels of Ts-SD,PPEI-APEI and QRS,improve the patients’ventricular pacing and percentage of ventricular perception,improve the body’s heart function and indicators of heart failure,which is worthy of clinical promotion.
