摘 要:目的:探讨CT增强扫描与平扫在胸部肿瘤三维适形放疗中的应用效果。方法:选取2017年1月-2019年12月就诊于本院的82例胸部肿瘤患者为研究对象。所有患者均行CT平扫与CT增强扫描,在CT增强扫描图像上勾画靶区与危及器官(OAR),将其靶区全部复制到CT平扫中,使CT平扫与CT增强扫描的OAR、靶区大小、位置、形态完全一致,分别在CT平扫与CT增强扫描下设计治疗计划。比较CT平扫与CT增强扫描下计划靶区(PTV)、肉眼肿瘤靶区(GTV)、危险器官剂量、近似最高剂量(D2%)、中位剂量(D50%)、近似最低剂量(D98%)、平均剂量(Dmean)及均匀指数(HI)。结果:CT增强扫描下PTV与GTV平均剂量均低于CT平扫(P<0.05);CT增强扫描与CT平扫下肺平均剂量、脊髓平均剂量、心脏平均剂量、脊髓最大剂量、D2%、D50%、D98%、Dmean及HI比较,差异均无统计学意义(P>0.05)。结论:与CT平扫相比,CT增强扫描可减少胸部肿瘤三维适形放疗中PTV与GTV剂量,但两者对危险器官剂量、D2%、D50%、D98%、Dmean及HI的影响无明显差异。Objective:To investigate the application effect of CT enhanced scan and plain scan in three-dimensional conformal radiotherapy for chest tumor.Method:A total of 82 patients with chest tumor admitted to our hospital from January 2017 to December 2019 were selected as the study subjects.All patients underwent plain CT enhanced scan and plain scan.The target area and organs at risk(OAR)were sketched on the CT enhanced scan image,and all the target areas were copied into CT plain scan,so that the OAR,target area size,position and shape of plain CT scan and enhanced CT scan were completely consistent.Treatment plans were designed under CT plain scan and CT enhanced scan,respectively.The planned target area(PTV),gross tumor target area(GTV),dangerous organ dose,approximate highest dose(D2%),median dose(D50%),approximate lowest dose(D98%),mean dose(Dmean)and homogeneity index(HI)were compared between CT plain scan and enhanced scan.Result:The average doses of PTV and GTV under CT enhanced scan were lower than those of CT plain scan(P<0.05).There were no significant differences in lung average doses,spinal average doses,heart average doses,spinal maximum doses,D2%,D50%,D98%,Dmean and HI between CT enhanced scan and CT plain scan(P>0.05).Conclusion:Compared with CT plain scan,CT enhanced scan could reduce the doses of PTV and GTV in three-dimensional conformal radiotherapy for chest tumors,but there was no significant difference in the effect of dangerous organ dose,D2%,D50%,D98%,Dmean and HI.
