关节镜下关节灌洗清理术治疗膝关节骨关节炎的效果及对患者VAS评分的影响

摘　　要：目的:探究关节镜下关节灌洗清理术治疗膝关节骨关节炎的效果及对患者VAS评分的影响。方法:选取2017年1月-2019年12月本院收治的67例膝关节骨关节炎患者,依据治疗方案将其分为对照组(n=33)和研究组(n=34)。对照组行常规治疗,研究组在关节镜下实施关节灌洗清理术。比较两组VAS评分、Lysholm膝关节功能评分、血清炎性指标水平变化情况,临床疗效及并发症发生情况。结果:研究组VAS评分低于对照组,而Lysholm膝关节功能评分高于对照组,差异均有统计学意义(P<0.05)。研究组临床疗效优于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清TNF-α、IL-6、IL-1、hs-CRP水平均低于治疗前,且研究组各指标水平均低于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:临床关节镜下关节灌洗清理术治疗膝关节骨关节炎效果较明显,可改善炎症反应与关节疼痛,且术后膝关节功能情况良好,术后并发症发生率较低,值得临床推广应用。Objective:To explore the effect of arthroscopic lavage debridement in the treatment of osteoarthritis of the knee joint and its influence on patients’VAS score.Method:A total of 67 patients with knee osteoarthritis admitted to our hospital from January 2017 to December 2019 were selected,they were divided into the control group(n=33)and the study group(n=34)according to the treatment plan.The control group received conventional treatment,while the study group underwent arthroscopic lavage debridement.VAS and Lysholm knee function scores,changes in serum inflammatory markers,clinical efficacy and complications were compared between the two groups.Result:VAS score in the study group was lower than that in the control group,while Lysholm knee function score was higher than that in the control group,with statistically significant differences(P<0.05).The clinical efficacy of the study group was better than that of the control group,the difference was statistically significant(P<0.05).After treatment,serum levels of TNF-α,IL-6,IL-1 and hs-CRP in the two groups were all lower than those before treatment,the levels of all indicators in the study group were lower than those in the control group,with statistically significant differences(P<0.05).The complication rate of the study group was lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Arthroscopic lavage debridement is effective in the treatment of osteoarthritis of the knee,which can improve inflammatory response and joint pain,besides,the function of the knee joint is better after surgery,the incidence of postoperative complications is low,it’s worthy of clinical promotion and application.
