硕通镜与输尿管硬镜治疗单侧输尿管结石的效果分析

摘　　要：目的:探讨负压组合式输尿管镜(硕通镜)与输尿管硬镜治疗单侧输尿管结石的效果。方法:选取2017年4月-2020年7月就诊于长治医学院附属和济医院的92例单侧输尿管结石患者的临床资料进行回顾性分析,按手术方式不同分为硕通镜组(n=55)与硬镜组(n=37)。硕通镜组进行硕通镜碎石术,硬镜组进行输尿管硬镜钬激光碎石术。比较两组术后1 d清石率、术后1个月清石率、Ⅰ期置鞘成功率、手术时间、术后住院时间及并发症发生情况。结果:两组Ⅰ期置鞘成功率比较,差异无统计学意义(P>0.05)。完成手术的病例中,硕通镜组手术时间短于硬镜组(P<0.05);两组术后住院时间比较,差异无统计学意义(P>0.05);硕通镜组术后1 d清石率为91.8%,高于硬镜组的75.0%(P<0.05);硕通镜组术后1个月清石率为95.9%虽高于硬镜组的86.1%,但差异无统计学意义(P>0.05)。硕通镜组并发症发生率低于硬镜组(P<0.05)。结论:采用硕通镜治疗单侧输尿管结石可提高术后1 d清石率,缩短手术时间,减少术后并发症,值得临床推广。Objective:To investigate the effect of negative pressure combined ureteroscopy(Shuo Tong ureteroscopy)and ureteroscopy on the treatment of unilateral ureteral calculi.Method:The clinical data of 92 patients with unilateral ureteral calculi in Heji Hospital Affiliated to Changzhi Medical College from April 2017 to July 2020 were retrospectively analyzed.They were divided into Shuo Tong ureteroscopy group(n=55)and ureteroscopy group(n=37)according to the different operation methods.The Shuo Tong ureteroscopy group was treated with Shuo Tong ureteroscopy lithotripsy and the ureteroscopy group was treated with ureteroscopy holmium laser lithotripsy.The stone removal rate 1 d after operation,stone removal rate 1 month after operation,success rate ofⅠstage sheath placement,operation time,postoperative hospital stay and complications were compared between the two groups.Result:There was no significant difference in the success rate ofⅠstage sheath placement between the two groups(P>0.05).Among the patients with completed the operation,the operation time in the Shuo Tong ureteroscopy group was lower than that in the ureteroscopy group(P<0.05).There was no significant difference in the postoperative hospitalization time of the two groups(P>0.05).The stone removal rate 1 d after operation of the Shuo Tong ureteroscopy group was 91.8%,higher than 75.0%of the ureteroscopy group(P<0.05).The stone removal rate 1 month after operation of the Shuo Tong ureteroscopy group was 95.9%,higher than 86.1%of the ureteroscopy group,but the difference was not statistically significant(P>0.05).The incidence of complications in the Shuo Tong ureteroscopy group was lower than that in the ureteroscopy group(P<0.05).Conclusion:The treatment of unilateral ureteral calculi by Shuo Tong ureteroscopy can improve the stone removal rate 1 d after operation,shorten the operation time and reduce postoperative complications,which is worthy of clinical promotion.
