改良乳晕菱形真皮瓣联合牵引矫正中重度乳头内陷的临床效果及预后分析

摘　　要：目的:探讨改良乳晕菱形真皮瓣联合牵引矫正中重度乳头内陷的临床效果及预后,为临床选择治疗方案提供参考。方法:选取2017年1月-2019年6月于本院接受治疗的中重度乳头内陷患者120例为研究对象。根据简单随机分组法将其分为观察组60例(118只乳头)和对照组60例(116只乳头)。观察组患者采用改良乳晕菱形真皮瓣联合牵引矫正治疗,对照组采用改良乳晕菱形真皮瓣治疗。比较两组患者治疗后的效果、乳头血运情况、并发症发生情况及患者对乳房外观的满意度。结果:观察组患者的治疗总有效率为99.15%,明显高于对照组90.52%,差异有统计学意义(P<0.05)。观察组乳头血运障碍发生率5.93%,低于对照组16.38%,差异有统计学意义(P<0.05)。观察组患者总并发症发生率6.78%,显著低于对照组20.69%,差异有统计学意义(P<0.05)。观察组患者的满意度为95.00%,高于对照组75.00%,差异有统计学意义(P<0.05)。结论:改良乳晕菱形真皮瓣联合牵引矫正中重度乳头内陷临床疗效确切,可作为治疗中重度乳头内陷的有效方法推广应用于临床治疗中。Objective:To investigate the clinical effect and prognosis of modified areola rhomboid dermal flaps combined with traction for correction of moderate and severe nipple inversion,and to provide reference for clinical treatment.Method:A total of 120 patients with moderate and severe inverted nipples who were treated in our hospital from January 2017 to June 2019 were selected as the study subjects.According to the simple random grouping method,60 patients(118 nipples)in the observation group and 60 patients(116 nipples)in the control group were divided.Patients in the observation group were treated with modified areola rhomboid dermal flap combined with traction correction,while patients in the control group were treated with modified areola rhomboid dermal flap.The results of treatment,blood flow to the nipple,complications and patients’satisfaction with breast appearance were compared between the two groups.Result:The total effective rate of patients in the observation group was 99.15%,significantly higher than 90.52%of the control group,the difference was statistically significant(P<0.05).The incidence of papillary dyslexia in the observation group was 5.93%,lower than 16.38%in the control group,with statistically significant difference(P<0.05).The incidence of total complications in the observation group was 6.78%,significantly lower than 20.69%in the control group,with a statistically significant difference(P<0.05).The satisfaction of patients in the observation group was 95.00%,higher than 75.00%of the control group,the difference was statistically significant(P<0.05).Conclusion:The modified areola rhomboid dermal flap combined with traction to correct moderate and severe inverted nipples has a definite clinical effect and can be widely used as an effective method for the treatment of moderate and severe inverted nipples.
