结核感染T细胞斑点试验、结核分枝杆菌抗体检查应用于肺结核患者诊断中的价值对比

摘　　要：目的:比较结核感染T细胞斑点(T-SPOT.TB)试验与结核分枝杆菌抗体(TB-Ab)检查对肺结核的临床诊断价值。方法:选取2018年6-12月本院收治的活动性肺结核患者80例作为肺结核组,其中涂阳肺结核34例,涂阴肺结核46例。另选取同期本院收治的非结核病患者40例作为对照组。两组均进行T-SPOT.TB试验与TB-Ab检查,比较两种检查方法对肺结核的诊断效能。比较肺结核组中涂阳、涂阴结核患者与对照组T-SPOT.TB试验斑点计数。结果:T-SPOT.TB试验诊断肺结核的灵敏度、特异度、诊断准确率、阳性预测值及阴性预测值均高于TB-Ab检查(P<0.05)。涂阳肺结核患者T-SPOT.TB试验A、B孔斑点计数均高于涂阴肺结核患者与对照组(P<0.05)。结论:与TB-Ab检查比较,T-SPOT.TB试验对肺结核的诊断价值更高;T-SPOT.TB试验斑点计数越多,则肺结核的活动程度可能越高。Objective:To compare the clinical value of T cell spot(T-SPOT.TB)test and mycobacterium tuberculosis antibody(TB-Ab)test in the diagnosis of tuberculosis.Method:A total of 80 patients with active tuberculosis admitted to our hospital from June to December 2018 were selected as the tuberculosis group,including 34 cases of smear positive tuberculosis and 46 cases of smear negative tuberculosis.In addition,40 non-tuberculosis patients admitted to our hospital during the same period were selected as the control group.Both groups were tested by T-SPOT.TB test and TB-Ab test to compare the diagnostic efficacy of the two methods in tuberculosis.The spot count of T-SPOT.TB test in tuberculosis patients with smear positive,negative and the control group were compared.Result:The sensitivity,specificity,diagnostic accuracy,positive predictive value and negative predictive value of T-SPOT.TB test were all higher than those of TB-Ab test(P<0.05).The spot count of T-SPOT.TB test A,B hole in smear positive tuberculosis patients were higher than those in smear negative tuberculosis patients and control group(P<0.05).Conclusion:Compared with TB-Ab test,T-SPOT.TB test has higher diagnostic value for tuberculosis;the more spots counted in T-SPOT.TB test,the more active tuberculosis may be.
