加速康复外科在妇科患者腹腔镜围手术期护理中的应用

摘　　要：目的探讨加速康复外科(ERAS)在妇科腹腔镜手术患者围手术期护理中应用效果。方法将200例患者均分为观察组和对照组,对照组给予常规护理,观察组在对照组基础上予以ERAS护理,比较两组患者术后24 h营养指标(前白蛋白、白蛋白、血尿素氮、血糖),应激指标(胰岛素抵抗指数、血清皮质醇浓度)水平,胃肠功能恢复指标(术后恢复排气和排粪时间)。结果两组患者术后24 h尿素氮(BUN)比较差异无统计学意义(P>0.05),其余营养指标组间比较差异有统计学意义(P<0.05);两组患者术后24 h应激指标比较,差异有统计学意义(P<0.05);观察组患者术后恢复排气时间、排便时间均早于对照组(P<0.05)。结论ERAS新理念在妇科腹腔镜手术患者围手术期实施可减轻患者应激反应,提高营养性指标水平,促进患者术后康复,值得临床推广应用。Objective To explore the application effect of enhanced recovery after surgery(ERAS)in perioperative nursing of patients undergoing gynecological laparoscopic surgery.Methods To equally divide 200 cases of patients into an observation group and a control group,the control group was given routine nursing care,and the observation group was given ERAS nursing care on the basis of the control group.The nutritional indicators(Prealbumin,albumin,blood urea nitrogen,blood glucose),stress indicators(insulin resistance index,serum cortisol concentration)levels,gastrointestinal function recovery indicators(postoperative recovery of exhaust and fecal time)of the two groups were compared 24 hours after operation.Results There was no statistically significant difference in serum urea nitrogen(BUN)between the two groups of patients at 24 h after surgery(P>0.05),and there are statistically significant differences in the comparison of other nutritional indicators between groups(P<0.05).In terms of the comparison of the stress indicators of the two groups of patients at 24 h after surgery,the difference was statistically significant(P<0.05).The recovery exhaust time and defecation time of patients in the observation group after operation were earlier than those in the control group(P<0.05).Conclusion The implementation of the new concept of ERAS in the perioperative period of gynecological laparoscopic surgery can reduce the patient's stress response,improve the level of nutritional indicators,and promote postoperative recovery.It was worthy of clinical promotion and application.
