粉尘螨滴剂舌下免疫治疗辅助鼻用糠酸莫米松治疗儿童变应性鼻炎的临床效果

摘　　要：目的:探讨粉尘螨滴剂舌下含服辅助鼻用糠酸莫米松鼻喷雾剂治疗儿童变应性鼻炎的临床疗效、预后及安全性。方法:随机选取160例尘螨过敏(中重度)的患儿(4~14岁)进行分组治疗,对照组和观察组各80例。在常规治疗的基础上,对照组给予粉尘螨滴剂舌下含服免疫治疗,观察组在对照组的基础上辅助鼻用糠酸莫米松鼻喷雾剂联合治疗,疗程2年。疗程结束后,比较两组临床疗效、临床症状积分和安全性。结果:观察组总有效率87.50%,高于对照组的63.75%(χ^2=2.209,P=0.028)。两组治疗前各鼻部症状评分比较,差异均无统计学意义(P>0.05);经过2年治疗后,两组各鼻部症状评分均明显下降,且观察组均明显低于对照组,差异均有统计学意义(P<0.05)。两组治疗前后RQLQ各维度得分比较,差异均无统计学意义(P>0.05)。结论:粉尘螨滴剂舌下含服辅助鼻用糠酸莫米松治疗儿童变应性鼻炎具有较高的临床疗效,能明显改善临床症状,复发率低,安全性较高。Objective:To investigate the clinical curative effect,prognosis and safety of Dermatophagoides Farinae Drops sublingual immunotherapy combined with co-administration Momethasone Furoate in the treatment of children with allergic rhinitis.Method:A total of 160 children with allergic rhinitis (moderate to severe,4-14 years ole) were randomly divided into the control group (Dermatophagoides Farinae Drops sublingual immunotherapy,n=80) and the observation group (on the basis of the control group,given co-administration Momethasone Furoate,n=80),two groups were given routine treatment.After treatment,the clinical efficacy,clinical symptoms and safety of two groups were evaluated.Result:The total effective rate of the observation group was 87.50%,it was higher than 63.75% of the control group (χ^2=2.209,P=0.028).There were no significant differences in the nasal-related symptom scores between two groups before treatment (P>0.05);after 2 years of treatment,the nasal-related symptom scores of two groups significantly decreased,and the observation group were significantly lower than those of the control group,the differences were statistically significant (P<0.05).There were no significant differences in the scores of RQLQ dimensions between two groups before and after treatment (P>0.05).Conclusion:Dermatophagoides Farinae Drops sublingual immunotherapy combined with co-administration Momethasone Furoate in the treatment of children with allergic rhinitis has a high clinical efficacy,it can significantly improve clinical symptoms,it has high security and low recurrence rate.
