B超引导下头皮神经阻滞复合全麻在神经外科麻醉中的应用

摘　　要：目的:探讨B超引导下头皮神经阻滞复合全麻在神经外科麻醉中的应用价值。方法:回顾性分析2019年1月-2020年3月本院收治的60例神经外科手术患者的临床资料,根据不同麻醉方式分为对照组和观察组,各30例。对照组行传统静脉全身麻醉,观察组行B超引导下头皮神经阻滞复合全麻。比较两组患者的麻醉效果、并发症发生情况、苏醒时间、术后不同时间点的疼痛程度。结果:观察组麻醉优良率为96.7%,明显高于对照组的73.3%,差异有统计学意义(P<0.05)。术后3、6 h,观察组视觉模拟法(VAS)评分明显低于对照组(P<0.05)。观察组苏醒时间为(10.3±2.2)min,明显短于对照组(13.5±2.6)min,差异有统计学意义(P<0.05)。观察组并发症发生率为3.3%,明显低于对照组的23.3%,差异有统计学意义(P<0.05)。结论:B超引导下头皮神经阻滞复合全麻在神经外科麻醉中的应用价值高,可增强麻醉效果,减轻疼痛程度,控制麻醉风险,值得临床推广。Objective:To investigate the application value of B-ultrasound guided scalp nerve block combined with general anesthesia in neurosurgical anesthesia.Method:The clinical data of 60 neurosurgery patients admitted to our hospital from January 2019 to March 2020 were retrospectively analyzed.According to different anesthesia methods,they were divided into the control group and the observation group,30 cases in each group.The control group underwent traditional intravenous general anesthesia,and the observation group underwent combined general anesthesia with scalp nerve block guided by B-ultrasound.The anesthesia effect,complications,recovery time,and pain degree at different time points after surgery were compared between two groups.Result:The excellent and good rate of anesthesia in the observation group was 96.7%,which was significantly higher than 73.3% of the control group,the difference was statistically significant (P<0.05).Visual analogue scale (VAS) score of the observation group at 3 and 6 h after surgery were significantly lower than those of the control group (P<0.05).The wake time of the observation group was (10.3±2.2) min,it was significantly shorter than (13.5±2.6) min of the control group,the difference was statistically significant (P<0.05).The incidence of complications was 3.3% in the observation group,which was significantly lower than 23.3% in the control group (P<0.05).Conclusion:Under the guidance of B-ultrasound,scalp nerve block combined with general anesthesia has a high application value in neurosurgical anesthesia,which can enhance the anesthetic effect,reduce the pain degree,and control the anesthetic risk.Therefore,it is worthy of clinical promotion.
