腹腔镜手术对早中期直肠癌患者胃肠功能恢复的影响效果研究

摘　　要：目的研究腹腔镜手术对早中期直肠癌患者胃肠功能恢复的影响效果。方法选取我院2019年1月~2019年12月收治的80例早中期直肠癌患者为研究对象,利用随机数字表法分为开腹组(40例)与腹腔镜组(40例),开服组采取开腹手术治疗,腹腔镜组采取腹腔镜手术治疗,对两组围手术期指标(手术时间、术中出血量、切口长度、手术费用、住院时间)、胃肠功能恢复指标(肠鸣音恢复时间、排气时间、排便时间)、肿瘤标志物(癌胚抗原、CA19-9、CA24-2)、并发症发生率进行对比。结果腹腔镜组手术时间长于开腹组、手术费用高于开腹组,术中出血量小于开腹组、切口长度短于开腹组、胃肠功能恢复指标优于开腹组,差异均有统计学意义(P<0.05);术前两组肿瘤标志物相比较,差异无统计学意义(P<0.05),术后与各组术前相比较,差异有统计学意义(P<0.05),腹腔镜组与开腹组相比较,差异无统计学意义(P>0.05);腹腔镜组并发症发生率5.00%,与开腹组20.00%相比较,差异有统计学意义(P<0.05)。结论早中期直肠癌患者行腹腔镜手术治疗有助于进一步提高胃肠功能恢复效果且并发症发生率低,值得加以推广使用。Objective To study the effect of laparoscopic surgery on the recovery of gastrointestinal function in patients with early and middle rectal cancer.Methods 80 patients with early and mid-term rectal cancer treated in our hospital from January 2019 to December 2019 were selected as the research subjects.They were divided into an open abdominal group(40 cases)and a laparoscopic group(40 cases)using random number table method.The open service group was treated with open surgery,and the laparoscopic group was treated with laparoscopic surgery.The perioperative indicators(operation time,intraoperative blood loss,incision length,surgical cost,length of hospital stay),and gastrointestinal function recovery indicators(The time of bowel sound recovery,exhaust time,defecation time),tumor markers(carcinoembryonic antigen,CA19-9,CA24-2),and the incidence of complications were compared.Results The operation time of the laparoscopic group was longer than that of the open group,and the operation cost was higher than that of the open group.The intraoperative blood loss was less than the open group,the incision length was shorter than the open group,and the index of gastrointestinal function recovery was better than that of the open group.Statistical significance(P<0.05);there was no significant difference in the tumor markers between the two groups before surgery(P<0.05),and there was statistically significant difference between the two groups before surgery(P<0.05)There was no significant difference between the laparoscopic group and the open group(P>0.05);The incidence of complications in the laparoscopic group was 5.00%,compared with 20.00%in the open group,and the difference was statistically significant(P<0.05).Conclusion Laparoscopic surgery for patients with early and mid-term rectal cancer is helpful to further improve the recovery of gastrointestinal function and the incidence of complications is low,which is worthy of popularization.
