结直肠癌息肉行EMR术后迟发性出血的相关因素分析

摘　　要：目的分析结直肠癌息肉行内镜粘膜切除术(EMR)术后迟发性出血的相关因素。方法选取2019年8月-2020年3月期间于我院165例行EMR治疗的结直肠癌息肉患者作为本研究对象。采用问卷调查形式统计患者性别、年龄、糖尿病、高血压、高血脂、动脉硬化等合并史、抗凝药物使用史、体质指数(BMI)、息肉大小、形状、位置、数量等资料。结果 165例患者中,EMR术后发生迟发性出血有15例,发生率为9.09%。单因素结果显示,患者性别、年龄、BMI、糖尿病、高血脂、动脉硬化、息肉位置及数量等指标与迟发性出血无关(P>0.05);高血压、使用抗凝药物、息肉大小及形状与迟发性出血密切相关(P<0.05)。经Logistic回归分析,患者合并高血压、使用抗凝药物、息肉直径≥1.5cm且有蒂为EMR术后迟发性出血的危险因素(P<0.05)。结论结直肠癌息肉患者合并高血压、使用抗凝药物、息肉大小及形状为EMR术后发生迟发性出血的危险因素,临床操作时需考虑并重视危险因素,降低术后出血风险。Objective To investigate the related factors of delayed bleeding after endoscopic mucosal resection(EMR)for colorectal cancer polyps.Methods 165 patients with colorectal cancer polyps underwent EMR in our hospital from August 2019 to March 2020 were enrolled,and were investigated by questionnaire,then the data including gender,age,diabetes,hypertension,hyperlipidemia,arteriosclerosis,history of anticoagulant use,body mass index(BMI),size,shape,location and quantity of polyps were collected.Results Among the 165 patients,15 had delayed bleeding after EMR,with an incidence rate of 9.09%.Univariate results showed that patients′gender,age,BMI,diabetes,hyperlipidemia,arteriosclerosis,location and quantity of polyps were not related to delayed bleeding(P>0.05),while hypertension,the use of anticoagulant drugs,the size and shape of polyps were closely related to delayed bleeding(P<0.05).Logistic regression analysis showed that hypertension,use of anticoagulant drugs,polyp diameter≥1.5 cm,and pedicle polyp were risk factors for delayed postoperative bleeding after EMR(P<0.05).Conclusion Hypertension,anticoagulant drugs,and the size and shape of polyps are risk factors for delayed bleeding after EMR in patients with colorectal cancer,so the above factors should be considered in clinical operation,so as to reduce the postoperative bleeding risk.
