摘 要:目的探讨膈肌超声评估慢性阻塞性肺疾病(COPD)严重程度的价值及膈肌超声与肺功能的关系。方法运用M型超声法、厚度法及面积法观察38例COPD患者的膈肌移动度、厚度及运动面积,根据肺功能检查结果将患者分为轻-中度组和重度组,采用ROC曲线判断膈肌超声评估重度COPD的效能。结果轻-中度组患者膈肌运动面积、用力呼吸时膈肌移动度均高于重度组(P<0.05)。面积法与M型超声法评估重度慢阻肺的ROC曲线下面积(AUC)分别为0.833和0.856;二者联合诊断的AUC为0.933。结论M型超声法与面积法可通过测量膈肌运动评估慢阻肺患者严重程度。Objective To explore the value of diaphragm ultrasonography in assessing the severity of chronic obstructive pulmonary disease(COPD)and the relationship between diaphragm ultrasonography and pulmonary function.Methods The diaphragm mobility,diaphragm thickness and diaphragm movement area of 38patients with COPD were observed by M-mode,B-mode ultrasound and the Area method.According to the results of pulmonary function test,the patients were divided into mild-moderate group and severe group.The ROC curve was used to judge the assessment effect of ultrasound for severe COPD.Results The movement area of diaphragm and the diaphragm mobility at maximal inspiratory effort in mild-moderate group were higher than those in severe group(P<0.05).The area under the ROC curve(AUC)of the Area method and M-mode ultrasonography was 0.833and 0.856respectively,and the AUC of combined diagnosis was 0.933.Conclusions M-mode ultrasound and the Area method can assess the severity of COPD patients by measuring diaphragm excursion.
