摘　　要：目的探讨经胸超声心动图(TTE)在儿童复合型先天性心脏病(CCHD)同期介入治疗中的应用价值。方法回顾性分析同期介入治疗112例CCHD的临床资料,TTE检查对比分析残余分流、跨肺动脉瓣压差、肺动脉高压、心脏腔室大小等变化情况。结果经TTE诊断的112例CCHD均成功行同期介入治疗。CCHD中以房间隔缺损(ASD)合并室间隔缺损(VSD)多见(29.5%),TTE随访中经皮球囊肺动脉瓣成形术(PBPV)术后97.2%跨肺动脉瓣压差恢复至正常,ASD合并VSD、VSD合并PDA者术后1个月LVDd较术前明显减小(P<0.05),99.1%无严重并发症发生。结论同期介入治疗儿童CCHD临床可行,TTE在其筛查、监测及随访中有重要的临床应用价值。Objective To investigate the value of transthoracic echocardiography(TTE)in simultaneous transcatheter intervention for children with compound congenital heart disease(CCHD).Methods This study retrospectively analyzed that a series of 112children diagnosed by TTE with CCHD in our institution and who received simultaneous transcatheter interventional therapy,the changes of residual shunt,transpulmonary gradient,pulmonary arterial hypertension,cardiac chamber size were compared and aralyzed.Results All patients successfully received simultaneous transcatheter therapy for CCHD.Atrial septal defect(ASD)combined with ventricular septal defect(VSD)was the most common type of CCHD(29.5%).The transpulmonary gradient returned to normal in 97.2%of the patients after percutaneous balloon pulmonary valvuloplasty(PBPV)detected by TTE.In patients with ASD combined with VSD and VSD combined with PDA,LVDd decreased significantly in the first month after intervention(P<0.05).No major adverse events occurred in 99.1%of the patients.Conclusions Simultaneous transcatheter intervention treatment of CCHD is clinically feasible,and TTE can be used as an effective imaging method for screening,monitoring and evaluation during procedure and follow-up.
