摘　　要：目的分析儿童消化道异位胰腺的超声表现。方法回顾分析20例经病理证实儿童消化道异位胰腺的超声表现,分析不同发生部位及病理类型之间的特征。结果20例消化道异位胰腺均为单发病灶,病变最大径线平均(1.7±0.5)cm,形状多为卵圆形,均匀等回声为主,内部均可见管状结构,血流信号中等为主。其中位于胃部4例,十二指肠5例,空肠5例,回肠6例。病理证实其中导管异位为主型4例,超声多普勒均为少许血流信号,腺泡异位为主型9例,超声多普勒为中等血流信号或丰富血流信号,混合型7例,超声多普勒为中等血流信号。13例患儿术前超声提示存在占位,其中1例诊断准确。结论儿童消化道异位胰腺临床症状不典型且发生部位多样,但超声表现存在一定特征,可在诊断中起到重要的作用。Objective To analyze the ultrasonographic manifestations of heterotopic pancreas in gastrointestinal tract of children.Methods The ultrasonographic features of 20children with ectopic pancreas in digestive tract confirmed by surgery and pathology were retrospectively analyzed.The characteristics between different occurrence sites and pathological types were analyied.Results All 20cases of ectopic pancreas in digestive tract were single lesions with the maximum averaging diameter(1.7±0.5)cm.The shape of the lesion was mostly oval,the echo was uniform and medium,and the tubular structure could be seen inside.Ultrasound Doppler was mainly medium blood flow signal.All the sites induded 4cases in stomach,5cases in duodenum,5cases in jejunum and 6cases in ileum.The pathology confirmed that there were 4cases of duct-based type,ultrasound Doppler was a little blood flow signal,acinarbased type was 9cases,ultrasound Doppler was moderate blood flow signal or abundant blood flow signal,mixed type 7cases,ultrasound Doppler was medium blood flow signal.Preoperative ultrasonography revealed occupancy in 13cases,of which one case was diagnosed accurately.Conclusions The clinical symptoms of heterotopic pancreas in digestive tract of children are atypical and the location of occurrence is varied,but the sonographic manifestations have certain characteristics,which can play an important role in the diagnosis of heterotopic pancreas in gastrointestinal tract.
