药师参与家庭医师团队开展药学服务的模式实践

摘　　要：目的探讨药师参与家庭医师团队开展药学服务模式的意义。方法遵照回顾性分析法纳入247例慢性疾病患者作为实验对象,其中150例患者接受药师参与的家庭医师团队服务,另97例接受常规的家庭医师团队服务,记录两组干预前后的药品费用支出、就诊及住院次数,并对其用药依从性、认知能力评分进行调查,最后发放满意度调查问卷表,对比两组服务结果。结果干预前两组的药品费用支出、就诊及住院次数相比无显著差异(P>0.05),干预后研究组各项指标均低于对照组(P<0.05)。干预前两组的用药依从性、认知能力评分相比无显著差异(P>0.05),干预后研究组各项评分均高于对照组(P<0.05)。研究组满意度明显高于对照组(P<0.05)。结论药学服务运用于家庭医师团队服务效果显著,可有效减少药品费用支出,降低住院就诊次数,提升患者配合用药的依从性,增强认知能力,满意度较高。Objective To study the clinical significance of family physician teams with involvement of pharmacists on pharmaceutical care.Methods 247 chronic disease patients were set as research subjects.Research group(150 cases)took the family physician team service with involvement of pharmacists;control group(97 cases)took the conventional family physician team service.Medical expenses,frequency of seeing a doctor and hospitalization,medication compliance,cognitive competence scores were recorded;patient’s satisfaction was assessed by questionnaire.Results Before intervention,medical expenses,and frequency of seeing a doctor and hospitalization between groups were not significantly different(P>0.05);after intervention,related indicators in the research group were lower than control group(P<0.05);before intervention,medication compliance and cognitive competence scores between groups were not significantly different(P>0.05);after intervention,related scores in the research group were higher than control group(P<0.05);nursing satisfaction in the research group was higher than control group(P<0.05).Conclusion The family physician teams with involvement of pharmacists can reduce the medical expenses and frequency of seeing a doctor and hospitalization,and improve patient’s medication compliance,cognitive competence and nursing satisfaction.
