摘 要:目的了解皮肤软组织感染患者的病原菌特征及耐药性,为临床合理用药提供理论依据。方法收集2014年1月-2018年12月宁波市第六医院皮肤软组织感染患者病灶标本,对其菌群分布及药敏结果进行回顾性分析。结果分离到病原菌192株,其中革兰阳性菌122株(63.54%),革兰阴性菌51株(26.56%),真菌19株(9.90%)。细菌检出前4位依次是金黄色葡萄球菌(66株,占34.38%),表皮葡萄球菌(34株,占17.71%),铜绿假单胞菌(16株,占8.33%),大肠埃希菌(15株,占7.82%)。分离的革兰阳性菌对青霉素和红霉素有较高的耐药率(>50%),表皮葡萄球菌对苯唑西林的耐药率高达88.24%。革兰阴性菌对氨苄西林、头孢唑啉、头孢曲松的耐药率较高(>50%)。结论引起皮肤软组织感染病原菌种类多样,临床应根据药敏结果合理选用抗生素进行治疗。Objective To understand the characteristics and drug resistance of pathogenic bacteria in patients with skin soft tissue infection,and to provide theoretical basis for clinical rational drug use. Methods Samples of lesions from patients with skin soft tissue infection in Ningbo Sixth Hospital from January 2014 to December 2018 were collected,and the distribution of bacterial flora and drug sensitivity results were retrospectively analyzed. Results A total of 192 strains of pathogenic bacteria were isolated,including 122 gram-positive strains( 63. 54%),51 gram-negative strains( 26. 56%) and 19 fungal strains( 9. 90%). The first four strains of bacteria were Staphylococcus aureus( 66 strains,34. 38%),Staphylococcus epidermodis( 34 strains,17. 71%),Pseudomonas aeruginosa( 16 strains,8. 33%) and Escherichia coli( 15 strains,7. 82%). The isolated gram-positive bacteria had high drug resistance to penicillin and erythromycin( > 50%),and the drug resistance rate of Staphylococcus epidermidis to benzoxicillin was 88. 24%. The resistance rate of gram-negative bacteria to ampicillin,cefazolin and ceftriaxone was high( > 50%). Conclusion The types of pathogenic bacteria causing skin soft tissue infection are various,and antibiotics should be selected reasonably according to the results of drug sensitivity.
