折叠悬挂式脑棉片支撑架在神经外科手术中的临床应用

摘  要:目的探讨脑棉片传统弯盘收集法与折叠悬挂式脑棉片支撑架收集法在神经外科手术中的临床应用效果。方法2017年1月至2018年12月行颅脑肿瘤手术50例,采用类随机分组方法,按择期手术日期先后顺序进行编号,以交替的方式分配到对照组25例采用脑棉片传统弯盘收集法和观察组25例采用折叠悬挂式脑棉片支撑架收集法。分别计算自颅内取出脑棉片并找到金属线时间、完整展开脑棉独立放置时间、弯盘凹槽内收集积血、血液污染手术敷料、脑棉片清点正确率和手术者满意度情况。结果2组自颅内找到金属线时间比较差异无统计学意义(P>0.05),观察组完整展开脑棉独立放置时间、积血收集量分别为(4.75±1.16)s、(25.00±0.57)ml,对照组分别为(12.94±4.55)s、(5.50±0.50)ml,2组比较差异有统计学意义(t值为27.12、5.28,均P<0.01);观察组血液污染敷料、脑棉片清点正确率、手术者满意度分别为12%(3/25)、100%(25/25)、100%(25/25),对照组分别为92%(23/25)、92%(23/25)、80%(20/25),2组比较差异有统计学意义(χ^(2)值为2.93、8.57、6.63,均P<0.01)。结论折叠悬挂式脑棉片支撑架是三维立体支架结构,操作支撑面稳定,充分空间利用,便于清点,积血不会遗留器械桌面且不会浸湿敷料巾,容器轻巧便于消毒。Objective To explore the clinical effect of the traditional curved disc collection method of the brain cotton sheet and the collection method of the folded hanging brain cotton sheet support frame in neurosurgery.Methods Fifty patients with craniocerebral tumor surgery were performed from January 2017 to December 2018.They were randomly grouped and numbered according to the date of theelective surgery.The control group was assigned to the control group in an alternating manner.25 patients were collected with traditional cotton discs.In the method and observation group,25 cases were collected by folding hanging brain cotton sheet support frame.The time taken to remove the brain cotton sheet from the skull and find the metal wire,the time for the complete deployment of the brain cotton,the accumulation of blood in the groove of the curved disk,the blood contamination surgical dressing,thecorrect rate of the brain cotton sheet inventory,and the satisfaction of the operator were calculatedl.Results There was no significant difference in the time of finding and intracranial metal wire between the two groups(P>0.01).The time for the independent deployment of the complete deployment of thebrain cotton in the observation group and the blood collection volume were(4.75±1.16)s and(25.00±0.57)ml,the control group was(12.94±4.55)s and(5.50±0.50)ml,the difference between the twogroups was statistically significant(t values were 27.12,5.28,both P<0.01);blood pollution dressing.brain cotton in the observation group .The accuracy of film count and the satisfaction rate of the operatorswere 12% (3/25).100% (25/25). and 100%e (25/25), respectively,and the control group was 92%(23/25)and 92%(23/ 25),80%(20/25),the difference between the two groups was statistically significant(χ^2 valucs were 2.93,8.57,6.63,all P <0.01).
