复合手术治疗硬脊膜动静脉瘘的疗效分析

摘　　要：目的探讨复合手术治疗硬脊膜动静脉瘘(SDAVF)的疗效。方法回顾性分析应用复合手术方法治疗的6例SDAVF患者的临床资料。手术前后采用改良的Aminoff-Logue评分(ALS)评判患者的脊髓功能。术后随访1~3个月,ALS评分达到优者为治愈,较术前下降,但未达到优者为改善,治愈和改善归为好转。结果本组患者中术前脊髓功能差者3例、良2例、中1例;平均改良ASL评分为7.67分。术后1~3个月随访,平均改良ASL评分为2.83分;其中治愈3例、好转3例,好转率达到100%。结论复合手术治疗SDAVF结合了传统手术与介入手术的优点,减少了手术创伤,降低了手术并发症,是一种值得临床推广的治疗方法。Objective To investigate the effect of combined surgical and endovascular for spinaldural arteriovenous fistula(SDAVF).Methods The clinical data of 6 SDAVF patients treated with combined surgical and endovascular were retrospectively analyzed.The modified aminoff-logue score(ALS)was used before and after the operation to evaluate the spinal cord function of patients.The follow-up period was 1-3 months.The patients with excellent ALS score were cured and decreased compared with those before operation,but those who did not reach the excellent score were regarded as improved,and the cured and improved patients were classified as improved.Results Spinal cord function was poor in 3 patients before operation,good in 2 patients and moderate in 1 patient.The improved ASL score was 7.67.The improved ASL score was 2.83 points in the follow-up of 1-3 months after operation.Of 6 patients,3 was cured and 3 were improved,the improvement rate reached 100%.Conclusion Hybrid of surgical and endovascular technique combines both advantages,reduces surgical invasion and complications,and worthy of popularization.
