摘 要:目的在健康扶贫背景下,评估四川省贫困农村地区儿童营养改善项目对儿童生长发育的影响。方法采用监测与横断面调查相结合的综合研究方法。收集2015-2019年四川省5岁以下儿童生长发育状况以及四川省儿童营养改善项目县儿童营养包服用情况的监测数据,对儿童生长发育及营养包服用情况的变化趋势进行描述性分析;在四川省儿童营养改善项目县中开展横断面现场调查,选取6~24月龄儿童及其看护人作为研究对象,通过问卷调查收集其社会人口学特征及儿童营养包服用情况,并通过测量儿童的身长和体重评估其生长发育状况。运用多因素logistic回归分析有效服用营养包对儿童生长发育的影响。结果监测数据显示,四川省5岁以下儿童的生长迟缓率由2015年的0.80%(2.64万/328.18万)下降至2019年的0.65%(2.19万/335.69万),低体重率由2.12%(3.97万/328.18万)下降至1.11%(3.71万/335.69万);儿童营养包有效服用率由2015年的89.55%(1.67万/1.86万)上升至2019年的91.18%(10.89万/11.94万)。横断面研究共纳入872对儿童及其看护人,回归分析显示,在控制相关混杂因素后,有效服用营养包能够降低儿童营养不良的风险(OR=0.61, 95%CI:0.43~0.90)。结论儿童营养改善项目在促进贫困农村地区儿童生长发育方面发挥了积极作用,有效服用营养包可降低儿童营养不良的风险。Objective To evaluate the impact of children's nutrition improvement projects in poor rural areas of Sichuan Province on children's growth and development in the context of health and poverty alleviation.Methods A comprehensive research method combining monitoring and cross-sectional surveys was used:(1)Collecting monitoring data on the growth and development of children under the age of 5 years old in Sichuan Province from 2015 to 2019 and the consumption of children's nutrition packs in the children's nutrition improvement project counties in Sichuan Province.Descriptive analysis of growth and development and changes in nutritional pack consumption.(2)Cross-sectional field surveys were carried out in Sichuan Children's Nutrition Improvement Project counties,and 6-24 months old children and their caregivers were selected as the research objects.Investigate and collect their sociodemographic characteristics and the consumption of children's nutrition packs,and evaluate the growth and development of children by measuring their length and weight.Multi-factor regression analysis was used to analyze the effect of effective nutrition pack on the growth and development of children.Results(1)Monitoring data showed that the growth retardation rate of children under the age of 5 years old in Sichuan Province decreased from 0.80%in 2015(26400/3.2818 million)to 0.65%(21900/3.3569 million)in 2019.2.12%(39700/3,281,800)decreased to 1.11%(37100/3,356,900).The effective use rate of child nutrition packs increased from 89.55%(16700/18.6 million)in 2015 to 91.18%(108900)in 2019/11.94 million.(2)A total of 872 pairs of children and their caregivers were included in the cross-sectional study.Regression analysis showed that after controlling for related confounding factors,effective consumption of nutrition packs can reduce the risk of child malnutrition(OR=0.61,95%CI:0.43-0.90).Conclusion The Children's Nutrition Improvement Project has played an active role in promoting the growth and development of children in poor rural areas.Ef
