摘 要:目的了解太原地区不同花粉血清特异性IgE阳性检出率的分布规律及其在变应性鼻炎伴或不伴哮喘患者中的阳性检出率。方法采用德国欧蒙优敏试剂盒和EUROBlotMaster II全自动免疫印记仪对2017年4月-2019年9月就诊于山西医科大学第一医院耳鼻咽喉头颈外科变态反应科门诊的变应性鼻炎患者进行树木组合(榆树、柳树、杨树)、艾蒿、豚草、葎草花粉反应特异性IgE检测及分析。结果共纳入1 221名患者,研究期间内花粉sIgE阳性人数分布呈双峰型,在3-5月有一个小高峰,在8-9月有一个大高峰;花粉过敏患者中,艾蒿血清sIgE呈阳性者占88.62%(1 082/1 221)。单一花粉血清sIgE阳性患者47.1%(575/1 221),其中艾蒿过敏患者最多37.8%(462/1 221);两种及两种以上花粉特异性IgE呈阳性者高达52.9%(646/1 221)。随着艾蒿sIgE表达水平增高,患者对两种及两种以上花粉血清sIgE的阳性率也随之增加(χ2=80.966,P<0.01)。结论艾蒿是引发该地区变应性鼻炎最主要的花粉过敏原,社会应加大对花粉过敏原的监测与防范,为花粉引起的过敏性疾病的预防及特异性免疫治疗提供参考依据。Objective To investigation the distribution of the specific IgE positive rate in different pollen serum in Taiyuan and its positive detection rate in patients with allergic rhinitis with or without asthma.Methods Patients with allergic rhinitis were observed in the outpatient department of Otorhinolaryngology Head and Neck Surgery,First Hospital of Shanxi Medical University from April 2017 to September 2019,their serum specific IgE of tree combinations(elm,willow,poplar),mugwort,ragweed,and humulus scandens pollen were analyzed by using German Ou Meng Youmin Kit and EUROBlotMaster II Automatic Imprinting Instrument.Results A total of 1221 patients were included in the study.Among the patients with pollen allergy,88.62%(1082/1221)showed positive IgE in the serum of mugwort.Patients with positive sIgE of single pollen serum accounted for 47.1%(575/1221),among which those with allergy to mugwort accounted for 37.8%(462/1221).Up to 52.9%(646/1221)of the serum had two or more pollen specific IgE positive.With the increase in the positive level of sIgE in mugwort,the positive rate of serum sIgE in two or more pollen patients also increased(χ2=80.966,P<0.01).Conclusion Mugwort is the most important pollen allergen in causing allergic rhinitis in this area,the society should increase the monitoring and prevention of pollen allergens,and provide reference for the prevention and specific immunotherapy of allergic diseases caused by pollen.
