摘　　要：目的探讨晚期获得性免疫缺陷综合征(acquired immunodeficiency syndrome,AIDS)患者抗病毒治疗后的生存状况及死亡影响因素。方法采用回顾性队列研究方法,选取昆明医科大学第一附属医院和建水县人民医院2014年1月—2019年9月收治的晚期AIDS患者2152例为研究对象,均接受抗病毒治疗,至随访截止时间死亡318例、存活1834例。对比不同生存状况患者性别、年龄、婚姻状况、文化水平、月收入情况、职业、感染途径、CD4+T细胞计数等基本资料差异,并进一步采用Logistics回归分析探讨其死亡的影响因素。结果吸毒、肿瘤、AIDS相关感染是晚期AIDS患者抗病毒治疗后死亡的前三位原因,分别占32.08%、20.75%、17.30%。Logistics回归分析发现:高龄、月收入较低、CD4+T细胞计数较低是晚期AIDS患者抗病毒治疗后死亡的危险因素(P<0.01)。结论晚期AIDS患者抗病毒治疗后死亡的影响因素较多,医生应综合各个因素,增加高危患者的干预强度以降低其病死率。Objective To investigate the survival status,influencing factors of mortality in advanced AIDS patients with antiviral therapy.Methods A retrospective cohort study was conducted on 2152 cases of advanced AIDS admitted to the First Affiliated Hospital of Kunming Medical University and People's Hospital of Jianshui County from January 2014 to September 2019 who were selected as study subjects and received antiviral therapy.By the end of study,318 patients died and 1834 survived.Gender,age,marital status,education level,monthly income,occupation,infection route,CD4+T cell count and other basic data of patients with different survival conditions were compared,and risk factors for death of advanced AIDS patients were analyzed by logistic regression.Results Drug abuse,tumor and AIDS-related infection were the top three causes of death in advanced AIDS patients who were treated with antiviral therapy,accounting for 32.08%,20.75%and 17.30%respectively.According to logistic regression analysis,advanced age,low monthly income and low CD4+T cell count were risk factors for mortality in patients with advanced AIDS undergoing antiviral therapy(P<0.01).Conclusion There are various influencing factors for the mortality of advanced AIDS patients undergoing antiviral therapy.Therefore,it is necessary for physicians to integrate various factors and increase the intensity of intervention of high-risk patients to reduce their mortality.
