摘 要:目的分析1型黏蛋白(MUC1)、白细胞介素-11(IL-11)在新生儿坏死性小肠结肠炎(NEC)肠黏膜组织中的变化及其意义,以期为NEC的防治提供依据。方法选取2015年2月-2019年7月郑州大学附属儿童医院病理科收集的手术后NEC患儿回肠末端肠黏膜组织65例(NEC组)、同期因先天性巨结肠实施手术治疗获取的小肠标本30例作为对照组;采用免疫组化染色检测两组标本中MUC1、IL-11蛋白的阳性表达率,并分析不同NEC病变分期组织中MUC1、IL-11蛋白阳性表达率差异。结果经免疫组化染色分析,NEC组患儿结肠黏膜组织中MUC1蛋白阳性表达率47.69%、IL-11蛋白阳性表达率53.85%,均低于对照组(80.00%、83.33%),差异有统计学意义(χ2=8.789、7.670,P<0.05);疾病分期为Ⅲ期的NEC组患儿结肠黏膜组织中MUC1蛋白阳性表达率33.33%、IL-11蛋白阳性表达率40.00%,均低于Ⅱ期患儿(60.00%、65.71%),差异均有统计学意义(χ2=4.605、4.298,P<0.05)。结论 NEC患儿肠黏膜组织中MUC1蛋白、IL-11蛋白表达强度均明显下调,在疾病分期越高的组织中下调表达越明显,因此推测这两种蛋白的降低表达可能参与了疾病的发生发展。Objective To investigate the changes and significance of type 1 mucin(MUC1) and interleukin-11(IL-11) in the intestinal mucosa of neonatal necrotizing enterocolitis(NEC), so as to provide evidence for the prevention and treatment of NEC in clinical practice. Methods From February 2015 to July 2019, a total of 65 cases of intestinal mucosal tissue samples of postoperative NEC children in Children′s Hospital of Zhengzhou University were collected and selected as NEC group, and 30 cases of colonic specimens obtained from surgical treatment of congenital megacolon during the same period were selected as the control group.Specimens of two groups were detected by immunohistochemical staining.The positive expression rates of MUC1 and IL-11 protein were analyzed and compared between the two groups. Results Immunohistochemical staining analysis indicated that the positive expression rates of MUC1 and IL-11 protein in colonic mucosa of children in NEC group were 47.69% and 53.85%,respectively, which were 80.00% and 83.33% in control group, and the difference was significant(χ~2 =8.789, 7.670, P<0.05).The positive expression rate of MUC1 and IL-11 protein in colonic mucosa were 33.33% and 40.00%, respectively, which were 60.00% and 65.71% in control group, and the difference was significant(χ~2 =4.605, 4.298, P<0.05). Conclusions The expression of MUC1 and IL-11 protein in the intestinal mucosa of NEC children are significantly down-regulated, and the expression is more significantly down-regulated in the tissues with higher disease stages.Therefore, it is speculated that the decreased expression of these two proteins may be involved in the development of NEC.
