摘　　要：目的评价吴茱萸择时选穴热熨对肝癌患者腹腔镜术后腹胀程度及胃肠功能恢复的影响。方法采用便利的抽样的方法,于2017年1月—2019年1月选择在本院肝胆胰外科住院治疗行腹腔镜肝切除术肝癌患者156例,按入院先后顺序编号,采用简单随机数字表法,分为观察组和对照组,每组各78例。术后观察组基于子午流注理论,选择大肠经、胃经(脾经)最旺盛的交汇点06:45-07:20这段时间内进行吴茱萸加粗盐热熨;对照组采用吴茱萸加粗盐热敷,整个过程30min,1次/d,连续3d。于干预前、术后24h、术后48h观察两组患者腹胀程度及两组患者肠鸣音恢复时间及首次肛门排气、排便时间的差异。结果广义估计方程分析显示,患者腹胀程度改善的时间效应、组间效应有统计学意义(P<0.001)。术后24h、术后48h观察组患者腹胀程度均较对照组低,差异有统计学意义(P<0.05)。观察组患者肠鸣音恢复时间及首次肛门排气、排便时间均较对照组短,差异有统计学意义(P<0.001)。观察组在治疗期间无出现不良反应。结论吴茱萸择时选穴热熨可有效改善肝癌患者腹腔镜术后腹胀程度和加速患者胃肠功能的恢复,并且是安全的。Objective To explore the effect of evodia hot compress on chosen acupoints at given time point on abdominal distension and recovery of gastrointestinal function in patients with liver cancer after laparoscopic surgery.Methods From January2017 to January 2019,a total of 156 patients with liver cancer undergoing laparoscopic surgery in our department were equally divided into control group and observation group upon their admission numbers by means of simple random number table.The latter was treated with evodia hot compress at 06:45 to 07:20,the most vigorous period of large intestine meridian and stomach meridian(spleen meridian)according to the theory of midnight-noon ebb-flow and the control group with ordinary evodia hot compress for 30 min once a day for 3 consecutive days.The degree of abdominal distension,the recovery time of bowel sounds and the time of first anal exhaust and defecation were observed before intervention,24 h and 48 h after operation.Results The degree of abdominal distension in the observation group at 24 h and 48 h after operation was significantly lower than that in the control group statistically(P<0.001).The recovery time of bowel sounds and the time of first anal exhaust and defecation in the observation group were both significantly shorter than those in the control group(P<0.05).Generalized estimation equation analysis showed that the time effect and inter-group effect of the improvement of abdominal distension were statistically significant(P<0.001).The observation group developed without adverse effect.Conclusion Evodia hot compress at given time points can effectively improve abdominal distension and accelerate the recovery of gastrointestinal function in patients with liver cancer after laparoscopic surgery.
