摘　　要：阐述社区慢性病管理的现状与存在的问题,从政策、技术和供需几方面分析慢性病互联网化管理体系搭建的可行性。建议构建“互联网+”社区慢性病管理体系,并探讨“互联网+”社区慢性病管理体系的架构、运行机制和保障机制。This paper first analyzes the state and the problems existing in the development of traditional community chronic disease management and then puts forward the feasibility of building an Internet-based chronic disease management system from the perspective of policy,technology and supply and demand.It proposes the construction of the Internet Plus community chronic disease management system and discusses the operation mechanism and guarantee mechanism of the system.
