摘　　要：目的:比较PFNA内固定与人工股骨头置换术治疗高龄股骨粗隆间骨折的效果。方法:2014年2月-2019年2月收治股骨粗隆间骨折老年患者53例,随机分为两组。A组采用PFNA内固定治疗;B组采用人工股骨头置换术治疗。比较两组治疗效果。结果:两组术后髋关节功能(Harris)评分及日常生活能力(ADL)评分比较,差异无统计学意义(P>0.05)。A组手术时间及术中出血量低于B组,差异有统计学意义(P<0.05);B组下床活动时间及并发症发生率均低于A组,差异有统计学意义(P<0.05)。结论:PFNA内固定与人工股骨头置换术治疗高龄股骨粗隆间骨折患者,均可达到良好的效果,PFNA内固定优势在于手术时间更短,术中出血量更少;人工股骨头置换术优势在于下床时间更早,降低因长期卧床导致的并发症发生率,有利于早期实施康复训练。Objective:To compare the effect of PFNA internal fixation and artificial femoral head replacement in the treatment of elderly patients with intertrochanteric fracture.Methods:From February 2014 to February 2019,53 elderly patients with intertrochanteric fractures were selected and randomly divided into two groups.Group A was treated with PFNA internal fixation.Group B was treated with artificial femoral head replacement.We compared the therapeutic effects of the two groups.Results:There was no significant difference in Harris score and ADL score after operation between the two groups(P>0.05).The operation time and blood loss in group A were lower than those in group B,the difference was statistically significant(P<0.05).The normal activity time and complication rate of group B were lower than those of group A,the difference was statistically significant(P<0.05).Conclusion:PFNA internal fixation and artificial femoral head replacement can achieve good results in the treatment of elderly patients with intertrochanteric fracture,PFNA internal fixation has the advantages of shorter operation time and less intraoperative blood loss.The advantages of artificial femoral head replacement are earlier time to get out of bed,and it can reduce the incidence of complications caused by long-term bed rest,and facilitate the early implementation of rehabilitation training.
