摘　　要：目的:探讨社区-家庭联动管理模式在中年高血压管理中的应用效果。方法:选取2018年8-10月在社区卫生服务中心签约的原发性高血压患者200例,随机分为两组。对照组接受常规门诊随访及健康教育;试验组采取社区-家庭联动管理模式。比较两组随访1年后血压控制率及服药依从性。结果:试验组随访1年后血压控制率优于对照组(以低危为例),服用依从率高于对照组,差异均有统计学意义(P<0.05)。结论:社区-家庭联动模式是开展社区中年高血压管理的有效方法。Objective:To explore the application effect of community family interaction model in the management of middle-aged patients with hypertension.Methods:From August to October 2018,200 patients with essential hypertension who signed up at the community health service center were selected and randomly divided into two groups.The control group received routine outpatient follow-up and health education.The experimental group adopted community family interaction management mode.We compared the blood pressure control rate and medication compliance of the two groups after 1 year follow-up.Results:In the experimental group,after one year of follow-up,the blood pressure control rate was better than that of the control group(taking low-risk as an example),and the medication compliance rate was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion:Community family interaction model is an effective way to carry out community middle-aged hypertension management.
