健脾化湿汤治疗非糖尿病高胰岛素血症临床观察

摘　　要：目的:观察健脾化湿汤治疗非糖尿病高胰岛素血症的临床疗效。方法:选取2017年8月至2019年4月上海市嘉定区中医医院门诊治疗的非糖尿病高胰岛素血症患者60例,随机分为对照组和治疗组,每组30例。两组患者均给予加强运动锻炼、合理饮食等基础治疗。对照组加用二甲双胍缓释片治疗,治疗组加用健脾化湿汤治疗。结果:治疗组有效率83.3%,对照组有效率66.7%,治疗组高于对照组,差异有统计学意义(P<0.05);两组体质量指数均低于治疗前,两组间比较无统计学差异(P>0.05);治疗组治疗后空腹胰岛素低于对照组,差异有统计学意义(P<0.05);两组治疗后餐后2小时胰岛素、三酰甘油、低密度脂蛋白均低于治疗前(P<0.05),组间比较无统计学差异;治疗组治疗后胰岛素抵抗指数、胰岛β细胞功能指数均低于对照组,差异有统计学意义(P<0.05)。结论:健脾化湿汤能有效减轻高胰岛素血症患者体质量指数、胰岛素水平、三酰甘油、低密度脂蛋白水平。Objective:To observe the clinical curative effect of Spleen-Fortifying Dampness-Resolving Decoction on non-diabetic hyperinsulinemia.Methods:A total of 60 patients with non-diabetic hyperinsulinemia treated in Outpatient Department of Jiading District Hospital of Traditional Chinese Medicine of Shanghai from August 2017 to April 2019 were randomly divided into the control group and the treatment group,with 30 cases in each group.Two groups of patients were all given basic treatment such as strengthening exercise,reasonable diet and so on.Then the control group was additionally treated with metformin sustained-release tablets,and the treatment group was additionally treated with Spleen-Fortifying Dampness-Resolving Decoction.Results:The effective rate of the treatment group was 83.3%,and that of the control group was 66.7%.The difference was statistically significant(P<0.05).The body mass index of each of the two groups was lower than that before treatment,and there was no significant difference between the two groups(P>0.05).After the treatment,fasting insulin in the treatment group was lower than that in the control group,and the difference was statistically significant(P<0.05).The levels of insulin,triglyceride and low-density lipoprotein 2 hours after the treatment in the two groups were lower than those before the treatment,and there were no significant differences between the two groups(P<0.05).After the treatment,the insulin resistance index and isletβ-cell function index in the treatment group were lower than those in the control group,and all the differences were statistically significant(P<0.05).Conclusion:Spleen-Fortifying Dampness-Resolving Decoction can effectively reduce body mass index,insulin level,triglyceride and low-density lipoprotein levels in patients with hyperinsulinemia.
