熄风健脾化痰法联合盐酸氟桂利嗪胶囊治疗前庭性偏头痛临床研究

摘　　要：目的:观察熄风健脾化痰法联合盐酸氟桂利嗪胶囊治疗前庭性偏头痛的临床疗效。方法:将60例前庭性偏头痛患者,按照随机数字表法分为治疗组和对照组,每组各30例。对照组口服盐酸氟桂利嗪胶囊治疗,治疗组在对照组的基础上以熄风健脾化痰法为治疗大法,给予中药汤剂口服治疗。比较两组患者的临床疗效及治疗前后视频头脉冲试验(video head impulse test,vHIT)、脑血流速度、眩晕障碍量表(dizziness handicap inventory,DHI)评分、中医证候积分变化情况。结果:两组患者治疗后vHIT检查结果改善情况优于本组治疗前,且治疗组治疗后优于对照组治疗后同期,差异具有统计学意义(P<0.05)。两组患者治疗后脑血流速度、DHI评分、中医证候积分均低于本组治疗前,且治疗组治疗后低于对照组同期治疗后,差异具有统计学意义(P<0.05)。对照组有效率为86.7%,治疗组有效率为93.4%,两组患者有效率比较,差异具有统计学意义(P<0.05)。结论:熄风健脾化痰法联合盐酸氟桂利嗪胶囊治疗前庭性偏头痛,可改善患者的临床症状及脑血流速度。Objective:To observe the clinical curative effect of method of extinguishing wind and activating spleen and resolving phlegm combined with Flunarizine Hydrochloride Capsule on vestibular migraine.Methods:A total of 60 patients with vestibular migraine were randomly divided into the treatment group and the control group,with 30 cases in each group.The control group was treated with Flunarizine Hydrochloride Capsule,while the treatment group was additionally treated with TCM decoction on the basis of the control group regarding method of extinguishing wind and activating spleen and resolving phlegm as the main principle.Then the clinical efficacy and changes of video head impulse test(vHIT),cerebral blood flow velocity,dizziness handicap inventory(DHI)score and TCM syndrome score before and after the treatment were compared between the two groups.Results:After the treatment,the improvement of vHIT results of the two groups was better than that of the same group before the treatment,and the improvement of the treatment group was better than that of the control group after the treatment at the same period,and the difference was statistically significant(P<0.05).The cerebral blood flow velocity,DHI score and TCM syndrome score of the two groups were lower than those of the same group before the treatment,and the above indexes in the treatment group were lower than those in the control group after the same period of treatment,and all the differences were statistically significant(P<0.05).The effective rate of the control group was 86.7%,and that of the treatment group was 93.4%.The difference was statistically significant(P<0.05).Conclusion:In terms of vestibular migraine,method of extinguishing wind and activating spleen and resolving phlegm combined with Flunarizine Hydrochloride Capsule can improve the clinical symptoms and cerebral blood flow velocity of the patients.
