摘　　要：目的:通过系统评价及累积Meta分析评价针灸治疗原发性高血压的有效性,为临床推广应用提供依据。方法:检索中国生物医学文献光盘数据库(CBM)、中国期刊全文数据库(CNKI)、万方资源数据库、维普数据库。检索时限为建库至2017年12月,根据情况采取主题词和自由词检索相结合的方式。结果:共检出1902篇文献,经过三次筛选最终纳入16篇文献。在纳入的16篇文献中,通过Jadad评分质量由高到低,3分6篇,2分10篇。针灸治疗原发性高血压疗效优于对照组,有效率差异具有统计学意义(OR=2.74;95%CI[1.79,4.18];P<0.00001)。针灸最初被证实对本病有效的时间为2002年[OR=3.82,95%CI[1.15,12.71]];后续研究加入均表现为95%CI长度的缩窄,表明总体治疗效应值的精确性逐步增加。结论:针灸治疗原发性高血压长期疗效优于西药,总体治疗效应值的精确性逐步增加,但疗效稳定性可能存在变异性。Objective:To evaluate the effectiveness of acupuncture and moxibustion in the treatment of primary hypertension by systematic evaluation and cumulative meta-analysis,and to provide basis for clinical application.Methods:CBM,CNKI,Wanfang Resources Database and Weipu Database were searched.The retrieval time was from the establishment of the database to December 2017.According to the situation,the combination of subject words and free words retrieval was adopted.Results:A total of 1902 papers were retrieved and 16 papers were finally included after three screenings.Among the 16 articles included,the quality of Jadad scores ranged from high to low,with 3 points in 6 articles and 2 points in 10 articles.The curative effect of acupuncture and moxibustion in the treatment of primary hypertension was better than that of the control group,and the difference was statistically significant[OR=2.74;95%CI(1.79,4.18);P<0.00001].Acupuncture and moxibustion were initially proved to be effective for this disease in 2002[OR=3.82,95%CI(1.15,12.71)].Subsequent studies showed that the length of 95%CI was narrowed,indicating that the accuracy of the overall therapeutic effect value increased gradually.Conclusion:Acupuncture and moxibustion is effective in the treatment of primary hypertension,and its long-term clinical efficacy is better than that of Western medicine.The accuracy of the total therapeutic effect value gradually increases,but the stability of the therapeutic effect may be variable.
