健脾祛湿和络方治疗慢性肾炎脾虚湿热证

摘　　要：目的:评价健脾祛湿和络方对慢性肾炎患者蛋白尿的影响。方法:采用平行、随机、阳性药物对照方法,将慢性肾炎脾虚湿热证患者89例分为两组。治疗组给予健脾祛湿和络方治疗,对照组给予氯沙坦钾治疗,每天50~100 mg,治疗24周。主要观察指标为24小时尿蛋白定量、尿红细胞数、中医证候积分及临床疗效。结果:治疗24周后,治疗组有效率为76.19%,对照组为24.4%,治疗组显著高于对照组(P<0.05)。治疗8周后,治疗组24小时尿蛋白定量显著低于治疗前(P<0.05),治疗8周、16周、24周后呈显著下降趋势。治疗16周、24周后,治疗组24小时尿蛋白定量显著低于对照组(P<0.05)。两组治疗后尿红细胞数较治疗前有所下降,但差异无统计学意义,且两组治疗后比较,差异亦无统计学意义。治疗后,两组中医证候积分均降低,且治疗24周后,治疗组显著低于对照组(P<0.05)。两组治疗过程中无不良反应。结论:健脾祛湿和络方能够有效减少慢性肾炎脾虚湿热证患者尿蛋白水平,且安全性良好。Objective:To evaluate the effect of Jianpi Qushi Heluo Prescription on chronic nephritis proteinuria.Methods:Parallel,random,and positive drug control was used to treat 89 patients with chronic nephritis with spleen deficiency and Damp-heat syndrome.The treatment group was treated with Jianpi Qushi Heluo Prescription,and the control group was treated with Losartan potassium 50-100 mg/day for 24 weeks.The main observation indexes were urine protein quantification,urine red blood cell count,and TCM syndrome score.Results:After 24 weeks of treatment,the effective rate was 76.19%in the treatment group and 24.4%in the control group.The treatment group was significantly higher than the control group.After 8 weeks of treatment,the quantitative urine protein in the treatment group was significantly lower than before treatment,and showed a significant downward trend after 8 weeks,16 weeks,and 24 weeks of treatment.After 16 weeks and 24 weeks of treatment,the quantitative urine protein of the treatment group was lower than that of the control group.After treatment,the urine red blood cells of the two groups decreased compared with before and after treatment.There was no statistically significant difference between the two groups after treatment.After treatment,the scores of TCM syndromes in both groups were reduced,and after 24 weeks of treatment,the treatment group was lower than that of the control group.There were no adverse reactions during the treatment of the two groups.Conclusion:Jianpi Qushi Heluo Prescription can effectively reduce the urine protein level of patients with chronic nephritis with spleen deficiency and Damp-heat syndrome,with sufficient safety.
