摘  要:目的探讨痰热清注射液对慢性阻塞性肺疾病(COPD)模型大鼠肺组织超微结构和炎性因子的影响。方法采用烟熏+脂多糖经气道滴定法进行COPD造模,造模时间为8周。将48只COPD模型大鼠按随机数字表法平分为3组:对照组、痰热清组、沙丁胺醇组,分别给予0.9%氯化钠注射液、痰热清注射液与沙丁胺醇注射液治疗,1次/d,连续2周。每组在治疗第1周与第2周分别处死8只大鼠,记录3组大鼠肺功能[第0.3秒用力呼气容积与用力肺活量的百分比(FEV0.3/FVC)、呼气中期流速(EF50)]、肺组织超微结构和炎性因子[白细胞介素-6(IL-6)、白细胞介素-8(IL-8)]表达变化情况。结果痰热清组、沙丁胺醇组大鼠治疗第1周与第2周的FEV0.3/FVC、EF50值均显著高于对照组,痰热清组显著高于沙丁胺醇组,差异有统计学意义(P<0.05)。痰热清组、沙丁胺醇组治疗第1周与第2周的血清IL-6、IL-8含量均显著低于对照组,痰热清组显著低于沙丁胺醇组,差异有统计学意义(P<0.05)。痰热清组、沙丁胺醇组治疗第1周与第2周的黏膜炎性细胞聚集炎症评分均显著低于对照组,痰热清组显著低于沙丁胺醇组,差异有统计学意义(P<0.05)。电镜下可见对照组大鼠肺组织肌层上皮纤毛黏连甚至脱落,肺泡壁结构变薄甚至断裂;痰热清组、沙丁胺醇组的大鼠肺组织病理均较对照组明显减轻,管腔内分泌物有所减少,有部分纤毛功能恢复。结论痰热清注射液在COPD模型大鼠中的应用能改善肺组织超微结构,抑制炎性因子的释放,改善大鼠的肺功能。Objective To investigate the effects of Tanreqing injection on the ultrastructure and inflammatory factors of lung tissue in chronic obstructive pulmonary disease(COPD)model rats.Methods The COPD model was made by smoke+lipopolysaccharide via airway titration,and the model time was 8 weeks.48 COPD model rats were randomly equally divided into three groups-control group according to the number table method,Tanreqing group,and salbutamol group.They were given sodium chloride injection,Tanreqing injection and salbutamol injection respectively,1 time/d,for 2 consecutive weeks.8 rats in each group were sacrificed in the first week and the second week of treatment.And the lung function[the percentage of forced expiratory volume at 0.3 second to forced vital capacity(FEV0.3/FVC),mid-expiratory flow rate(EF50)],lung tissue ultrastructure and expression of inflammatory factors[interleukin 6(IL-6),interleukin 8(IL-8)]in the three groups of rats was recorded.Results The FEV0.3/FVC and EF50 values of the Tanreqing group and the salbutamol group were significantly higher than those of the control group in the first and second weeks of treatment,and the Tanreqing group was significantly higher than that of the salbutamol group(P<0.05).Serum IL-6 and IL-8 levels in the Tanreqing group and the salbutamol group were significantly lower than those in the control group in the first and second weeks of treatment,and the Tanreqing group was significantly lower than the salbutamol group(P<0.05).The mucosal inflammatory cell aggregation and inflammation scores of the Tanreqing group and the salbutamol group were significantly lower than those of the control group in the first and second weeks of treatment,and the Tanreqing group was significantly lower than that of the salbutamol group(P<0.05).Under the electron microscope,it can be seen that in the lung tissue of the control group,the muscular epithelial cilia adhered or even fell off,and the alveolar wall structure became thinner or even broken;Compared with the control group,the lun
