摘　　要：目的分析白细胞介素-37(IL-37)水平与骨关节炎(OA)疾病活动度及炎性介质肿瘤坏死因子-α(TNF-α)、血清白细胞介素-1β(IL-1β)、CXC趋化因子配体8(CXCL8)、人基质金属蛋白酶-3(MMP-3)的相关性。方法前瞻性选取2015年3月至2018年4月青海省人民医院骨科收治的骨关节炎患者96例(OA组)作为研究对象,选择年龄相仿体检的健康人60例作为对照组。观察2组疾病活动度评分(DAS28),检测2组血清C反应蛋白(CRP)、红细胞沉降率(ESR)、类风湿因子(RF)、IL-37及炎性介质TNF-α、IL-1β、CXCL8、MMP-3水平;评估IL-37与DAS28的相关性及炎性介质TNF-α、IL-1β、CXCL8、MMP-3的相关性。结果OA组DAS28评分、CRP、ESR、RF分别为(49.75±1.22)分、(64.58±9.74)mg/L、(69.74±10.63)mm/h、(468.52±11.51)IU/L,明显高于对照组[(1.08±1.17)分、(3.15±8.69)mg/L、(14.6±9.74)mm/h、(62.47±13.67)IU/L],差异有统计学意义(P<0.05);OA组血清IL-37、TNF-α、IL-1β、CXCL8、MMP-3水平分别为(217.17±4.87)ng/mL、(3.64±0.98)pg/mL、(7.29±1.08)pg/mL、(28.67±1.32)μg/L、(121.47±33.64)μg/L,显著高于对照组[(14.62±5.87)ng/mL(0.72±0.78)pg/mL、(2.19±0.97)pg/mL、(2.28±1.65)μg/L、(102.54±28.92)μg/L],差异有统计学意义(P<0.05);OA患者血清IL-37与DAS28呈正相关(r=0.497,P<0.05);IL-37和TNF-α、IL-1β、CXCL8及MMP-3呈正相关关系(r=0.352、0.547、0.494、0.330,P<0.05)。结论IL-37血清水平变化可影响骨关节炎的炎性介质TNF-α、IL-1β、CXCL8、MMP-3因子表达;IL-37与骨关节炎疾病活动度呈正相关关系,可作为评估疾病严重的程度的重要指标。Objective To investigate the correlation between interleukin-37(IL-37)levels and osteoarthritis(OA)disease activity and inflammatory mediators tumor necrosis factor-α(TNF-α),serum interleukin-1β(IL-1β),CXC chemokine ligand 8(CXCL8)and human matrix metalloproteinase(MMP-3).Methods Ninty-six cases of OA patients in Department of orthopaedics department(OA grroup)and 60 cases of heathy examinees(control group)in the Medical Examination Center of Qinghai Provincial People's Hospital from March 2015 to April 2018 were selected.The disease activity score(DAS28),serum C-reactive protein(CRP),erythrocyte sedimentation rate(ESR),rheumatoid factor(RF),IL-37 levels and inflammatory mediators including TNF-α,IL-1β,CXCL8 and MMP-3 were assessed and recorded.Then the correlation of IL-37 level and osteoarthritis disease activity and inflammatory mediators TNF-α,IL-1β,CXCL8 and MMP-3 was analyzed.Results The scores of DAS28,CRP,ESR and RF in the OA group were(49.75±1.22)points,(64.58±9.74)mg/L,(69.74±10.63)mm/h,and(468.52±11.51)IU/L,which were significantly higher than those of the control group[(1.08±1.17)points,(3.15±8.69)mg/L,(14.6±9.74)mm/h,(62.47±13.67)IU/L],the differences were statistically significant(P<0.05);the serum levels of IL-37,TNF-α,IL-1β,CXCL8,and MMP-3 in the OA group were(217.17±4.87)ng/mL,(3.64±0.98)pg/mL,(7.29±1.08)pg/mL,(28.67±1.32)μg/L,(121.47±33.64)μg/L,respectively,which were significantly higher than those of the control group[(14.62±5.87)ng/mL(0.72±0.78)pg/mL,(2.19±0.97)pg/mL,(2.28±1.65)μg/L,(102.54±28.92)μg/L],the differences were statistically significant(P<0.05).Serum IL-37 was positively correlated with DAS28,TNF-α,IL-1β,CXCL8,MMP-3(r=0.497,0.352,0.547,0.494,0.330,P<0.05).Conclusion Serum IL-37 levels can affect the inflammatory mediators including TNF-α,IL-1β,CXCL8 and MMP-3.IL-37 is positively correlated with the disease activity of osteoarthritis,and can be used as an important indicator to assess the severity of the disease.
