摘 要:目的探讨新型头颈固定支架在颈椎后路手术中的临床应用价值。方法2018年8月—2019年12月接受颈椎后路手术患者40例,术中应用新型头颈固定支架固定20例(试验组),应用Mayfield头架固定20例(对照组),比较2组患者手术前后颈椎曲度指数(CCI)及术后头面部皮肤受损情况。结果所有手术均顺利完成。试验组CCI术前为(13.58±2.82)%,术后为(18.26±3.01)%;对照组CCI术前为(13.47±3.01)%,术后为(16.31±2.75)%;2组术后CCI较术前均明显改善,且试验组改善程度优于对照组,差异均有统计学意义(P<0.05)。试验组患者皮肤完好18例,轻度受压2例;对照组皮肤完好17例,头皮出血1例,头皮拉伤2例;组间比较差异无统计学意义(P>0.05)。结论新型头颈固定支架操作简便,固定牢靠,能更好地保护颈椎正常生理曲度及头面部皮肤,是一种适应多种颈椎后路手术的较理想的新型支架,值得临床推广。Objective To explore the clinical application value of a new type head and neck fixtion stent in posterior cervical surgery.Methods From August 2018 to December 2019,40 patients underwent posterior cervical surgery,among them 20 were fixed with the new type head and neck fixator(experiment group),and 20 with Mayfield head frame(control group).The pre-and post-operative cervical curvature index(CCI)and postoperative scalp and facial skin damage were compared between the 2 groups.Results All the operations were completed successfully.The CCI of the experiment group was(13.58±2.82)%before operation and(18.26±3.01)%after operation.The CCI of the control group was(13.47±3.01)%before operation and(16.31±2.75)%after operation.The CCI of the 2 groups was significantly improved after operation compared with that before operation,and the improvement degree of the experiment group was better than that of the control group,all with a statistically significant difference(P<0.05).In the experiment group,the skin was intact in 18 cases,slightly compressed in 2;in the control group,the skin was intact in 17,scalp hemorrhage in 1 and scalp injury in 2;and there was no statistical difference between the 2 groups(P>0.05).Conclusion The new head and neck fixation stent is easy to operate and can get firm fixation,thus can better protect the normal physiological curvature of cervical vertebra.It is an ideal new type of stent for various cervical vertebrae posterior surgeries and worthy of clinical popularization.
