摘　　要：目的探讨桡侧副动脉穿支皮瓣修复虎口缺损型创面的临床应用及解剖基础。方法应用桡侧副动脉穿支皮瓣游离移植修复虎口创面患者27例,均为Ⅱ期修复,缺损面积:4.0 cm×3.0 cm^17.0 cm×7.0 cm,术中记录穿支数量及穿支浅出点的分布,随访观测皮瓣及供区外形、血运、感觉及运动功能恢复。结果本组27例患者皮瓣均成活。术后随访6~18个月,平均11个月,皮瓣外观及质地良好,2例皮瓣略显臃肿。6个月后行皮瓣修薄,术后效果满意。患者均无溃疡发生,感觉恢复至S3级以上,供区均行I期闭合,肢体活动无障碍。结论桡侧副动脉穿支皮瓣修复虎口缺损型创面质地良好,血供可靠,临床效果满意,值得推广应用。Objective To investigate the clinical effects and basal anatomy of radial collateral artery perforator flap repairs soft tissue defects in the first web.Methods A total of 27 cases of first web wounds were repaired by free transplantation of radial collateral artery perforator flaps,all of which were repaired in stage II.The defect area was 4.0 cm×3.0 cm-17.0 cm×7.0 cm.The number and the distribution of the superficial exit points of perforator branches during the operation was recorded.The shape,blood supply,sensory and motor function of the flap and donor area were followed up.Results All the 27 flaps survived.These cases were followed up for 6-18 months after the surgery,with an average of 11 months.All flaps had good appearance and texture,two flaps were slightly swollen.Thinned surgery made the postoperative result satisfactory after 6 months.There were no ulcer cases,and the sensation returned to level S3 or above.All the donor areas were closed in stage I,and the limb movement was barrier-free.Conclusion The radial collateral artery perforator flap has good texture,reliable blood supply and satisfactory clinical effects in repairing the defect of the first web and are worthy of popularization and application.
