战创血管伤复合手术治疗的围手术期护理

摘　　要：目的总结颈部及邻近躯干大血管损伤病人复合手术治疗围手术期的护理经验。方法回顾性分析1978~2014年复合手术治疗的42例颈部及邻近躯干大血管损伤的临床资料。“放风筝”法治疗3例,“血管内”止血带辅助手术治疗35例,开放手术与介入手术先后实施治疗3例,根据手术方法采取针对性护理措施。结果42例均治愈,无手术死亡或因治疗加重残废。结论复合手术治疗战创血管损伤对护理提出了新的较高要求,协助医生完成术前评估,做好术中配合、术后观察,可以预防并发症,实施周密的护理措施是保证病人康复的基础。Objective To summarize the comprehensive nursing measures in the perioperative period of hybrid operation for the patients with traumatic injuries of large vessels in neck or regions adjacent to trunk.Methods The clinical data of 42 patients with traumatic injuries of large vessels in neck or regions adjacent to trunk who undwewent hybrid operation from 1978 to 2014 were analyzed retrospectively.The"flying kite"method was used in 4 patients.The"intravascular"tourniquet assisted surgical removal of pseudoaneurysms and arteriovenous fistulas was used in 35 patients.Interventional therapy combined with open surgery was used in 3 patients.The comprehensive nursing measures were used according to the surgical method.Results All the 42 patients were cured.There was no death or disability due to the treatment.Conclusions It has put forward new and higher requirements on the perioperative nursing for the patients with traumatic injuries of large vessels in neck or regions adjacent to trunk undergoing hybrid operation.To assist the doctors to complete the preoperative evaluation,and good intraoperative nursing cooperation and postoperative observation are helpful to preventing postoperative complications.Comprehensive nursing is the basis to ensure the recovery of the patients.
