风险管控策略在急性胸痛患者院前急救护理应用效果

摘　　要：目的探讨风险管控策略在急性胸痛患者院前急救护理应用效果。方法将我院2018年1月至12月院前急救中心接收的以急性胸痛为主诉的急诊患者260例作为对照组,2019年1月至12月接收的273例作为观察组。对照组实施常规院前急救医疗服务(Pre-EMS),观察组采用基于风险管控的Pre-EMS,比较两组Pre-EMS派车时间、出车时间、处理时间、转运时间;分析两组急性胸痛患者并发症、预后及护理投诉率。结果观察组Pre-EMS过程中的派车时间、出车时间、处理时间、转运时间均显著低于对照组(P均<0.01);观察组急性胸痛患者的心力衰竭、心源性休克、心律失常及院内死亡率均显著低于对照组(P均<0.05);观察组护理投诉率显著低于对照组(P<0.05)。结论风险管控策略应用于急性胸痛患者的Pre-EMS,可有效缩短急救相关反应时间,降低并发症发生率、病死率及护理投诉率。Objective To explore the efficacy of risk control strategy in pre-hospital emergency nursing of patients with acute chest pain.Methods 533 patients with acute chest pain as the main complaint in our hospital emergency center from January 2018 to December 2019 were divided intocontrol groupand research group.The control group(n=260)received conventional pre-hospital emergency medical services(Pre-EMS),and the research group(n=273)were treated with Pre-EMS based on risk control strategies.The Pre-EMS dispatch time,departure time,processing time,transit time,and compatients,prognosis and nursing complaint rate of patients with acute chest pain were recorded and compared between the two groups.Results The dispatch time,departure time,processing time and transit time of Pre-EMS in study group were significantly lower than those of control group(P<0.01).The incidence rate of heart failure,cardiogenic shock,arrhythmia,and the hospital mortality rate were significantly lower than those of control group(P<0.05),the nursing complaint rate of the study group was significantly lower than that of control group(P<0.05).Conclusion The risk control strategy can effectively shorten the response time of first aid,reduce the incidence of complications,mortality and nursing complaints in Pre-EMS of patients with acute chest pain.
