摘　　要：目的:研究信息-动机-行为技巧(IMB)联合综合护理对冠心病PCI治疗患者自我管理、负面情绪管理及预后的影响。方法:于我院行PCI的105例冠心病患者随机被分为常规护理组(50例)和联合护理组(55例,在常规护理组基础上接受IMB联合综合护理)。两组均干预6个月。观察比较两组干预前后压力感知量表(PSS)、冠心病自我管理量表(CSMS)、焦虑自评量表(SAS)、抑郁自评量表(SDS)评分及术后6个月内心脏不良事件发生率。结果:干预后,与常规护理组比较,联合护理组PSS[(16.60±3.74)分比(10.18±1.95)分]、SAS[(44.20±5.28)分比(31.78±4.96)分]、SDS[(45.11±5.01)分比(33.09±5.31)分]评分降低更显著;心脏不良事件发生率(32.00%比9.09%,P=0.003)显著降低;CSMS的日常生活管理[(24.77±3.47)分比(32.24±2.60)分]、疾病医学管理[(49.38±5.29)分比(60.87±6.11)分]、情绪认知管理子量表评分[(10.45±2.03)分比(14.94±2.40)分]升高更显著(P均=0.001)。结论:IMB联合综合护理能显著降低冠心病PCI治疗患者的压力感知,提高自我管理水平,缓解焦虑、抑郁情绪,降低心脏不良事件发生率。Objective:To study influence of information-motivation-behavioral skill(IMB)combined comprehensive nursing on self-management,negative emotion management and prognosis in patients with coronary heart disease(CHD)undergoing PCI.Methods:A total of 105 CHD patients undergoing PCI in our hospital were randomly divided into routine nursing group(n=50)and combined nursing group(n=55,received IMB combined comprehensive nursing based on routine nursing group).both groups were intervened for six months.Scores of perceived stress scale(PSS),coronary artery disease self-management scale(CSMS),self-rating anxiety scale(SAS)and self-rating depression scale(SDS)before and after intervention,and incidence rate of adverse cardiac events six months after intervention were observed and compared between two groups.Results:Compared with routine nursing group,after intervention,there are significant reductions in scores of PSS[(16.60±3.74)scores vs.(10.18±1.95)scores],SAS[(44.20±5.28)scores vs.(31.78±4.96)scores]and SDS[(45.11±5.01)scores vs.(33.09±5.31)scores],incidence rate of adverse cardiac events(32.00%vs.,9.09%P=0.003)and significant rise in scores of daily life management subscale[(24.77±3.47)scores vs.(32.24±2.60)scores],disease medical management subscale[(49.38±5.29)scores vs.(60.87±6.11)scores],emotion cognitive management subscale[(10.45±2.03)scores vs.(14.94±2.40)scores]of CSMS in combined nursing group(P<0.01 all).Conclusion:IMB combined comprehensive nursing can significantly reduce stress perception,improve self-management,relieve anxiety and depression,and reduce incidence rate of adverse cardiac events in CHD patients undergoing PCI.
