利伐沙班联合益气抗栓丸治疗下肢深静脉血栓的临床疗效及对凝血功能影响

摘　　要：目的观察利伐沙班联合益气抗栓丸治疗下肢深静脉血栓的临床疗效并分析其对凝血功能的影响。方法选取2019年1月至12月在四川省骨科医院治疗的骨科术后下肢深静脉栓塞患者103例,给予利伐沙班治疗3个月后根据患者是否还有血栓及D-二聚体(DD)结果将其分为正常组(n=44)和异常组(n=59)。正常组再随机分为正常对照组(n=24)和正常研究组(n=20),正常对照组停药3个月再进行复查,正常研究组采用益气抗栓丸继续治疗3个月。异常组再随机分为异常对照组(n=27)和异常研究组(n=32),异常对照组继续利伐沙班治疗3个月,异常研究组采用利伐沙班联合益气抗栓丸继续治疗3个月。除正常对照组治疗时间为3个月,其余各组患者均连续治疗6个月。分别于治疗前、治疗3个月、治疗6个月观察各组临床疗效、肝肾损伤及出血情况,并于治疗前、治疗3周、治疗3个月、治疗6个月检测各组凝血功能指标(FIB、FDP、DD、FM)。结果治疗6个月后,正常对照组有效率及并发症情况与正常研究组差异无统计学意义。异常研究组并发症总体发生率显著低于异常对照组(P<0.05)。与正常组相比,异常组中FDP、DD水平无论是治疗前还是治疗后均显著高于正常组(P<0.05)。正常组和异常组中,治疗后FIB、FDP、DD、FM水平较治疗前显著下降(P<0.05),但正常对照组停药3个月后FM结果较停药前显著增加(P<0.05)。与异常对照组相比,利伐沙班联合益气抗栓丸治疗3个月后DD水平显著下降(P<0.05)。结论利伐沙班治疗DVT痊愈后,继续益气抗栓丸辅助治疗可在一定程度上预防血栓再次形成。同时,与单用利伐沙班相比,利伐沙班联合益气抗栓丸可有效改善下肢深静脉血栓患者发生并发症的几率,改善凝血指标,提高患者疗效。Objective To observe the clinical efficacy of rivaroxaban combined with Yiqi Kangshuan Pills to treat deep vein thrombosis of lower extremities and analyze its effect on coagulation function.Method A total of 103 patients with deep vein thrombosis after orthopedic surgery in our hospital were selected from January 2019 to December 2019.After 3 months of treatment with rivaroxaban,patients were divided into normal group(n=44)and abnormal group(n=59)according to whether they had thrombosis and D-dimer(DD)results.The normal group was randomly divided into a normal control group(n=24)and a normal study group(n=20).The normal control group was discontinued for 3 months before re-examination,and the normal study group was treated with Yiqi Kangshuan pills for 3 months.The abnormal group was randomly divided into abnormal control group(n=27)and abnormal study group(n=32).The abnormal control group continued rivaroxaban treatment for 3 months,and the abnormal study group was treated with rivaroxaban combined with Yiqi Kangshuan pills for 3 months.Except for the normal control group,the treatment time was 3 months,the other groups were treated for 6 months continuously.The clinical efficacy,liver and kidney damage,bleeding and the blood coagulation function indexes(FIB,FDP,DD,FM)of each group were observed before treatment,3 months of treatment,and 6 months of treatment.Results After 6 months of treatment,the effective rate and complications of normal control group were not significantly different from those of normal study group.The overall incidence of complications in abnormal study group was significantly lower than that in abnormal control group(P<0.05).Compared with normal group,FDP and DD in abnormal group were significantly higher than those in normal group both before and after treatment(P<0.05).In normal group and the abnormal group,the levels of FIB,FDP,DD,and FM after treatment were significantly lower than before treatment(P<0.05),but the results of FM in the normal control group were significantly higher aft
