摘　　要：目的探讨衡阳市肿瘤患者住院费用的构成情况及影响因素,为减轻肿瘤患者的疾病经济负担、改革医保付费方式提供参考。方法收集衡阳市两所三甲医院2016-2018年诊断为肺癌、胃癌、结肠癌、直肠癌和肝癌的病例数据。运用非参数检验、多重线性回归分析和决策树模型等统计分析方法,找出肿瘤住院患者费用的影响因素,并进行病例组合分类。结果多重线性回归模型具有统计学意义(P<0.05),住院天数、性别、年龄、是否手术和是否参保是肿瘤患者住院费用的影响因素,其中住院天数的影响程度最大。根据决策树模型,以上5个变量共产生了9个病例组合。结论通过科学合理地减少无效住院日、降低药占比、推动医保付费机制改革等措施,控制肿瘤患者住院费用的过快增长,可减轻患者经济负担。Objective To explore composition and influencing factors of hospitalization expenses in cancer patients in Hengyang City,and to provide reference for relieving the economic burden in cancer patients and reforming the payment mode of medical insurance.Methods Case data of lung cancer,gastric cancer,colon cancer,rectal cancer and liver cancer diagnosed in 2 level-Ⅲhospitals in Hengyang City from 2016 to 2018 were collected.Statistical analytical methods such as the non-parametric test,multiple linear regression analysis and the decision tree model were applied,the influencing factors of hospitalization expenses in cancer patients were found out,and case combination classification was carried out.Results the multiple linear regression model was statistically significant(P<0.05).The length of hospital stay,gender,age,surgery and insurance were the influencing factors of hospitalization expenses in cancer patients,and the hospital stay had the greatest influence.According to the decision tree model,the above 5 variables produced 9 case combinations.Conclusion Through reasonable decrease of invalid hospital stay,decrease of the proportion of drugs,promotion of reforming the payment mechanism of medical insurance and control on excessive growth of hospitalization expenses in cancer patients,the economic burden of patients can be relieved.
