摘  要:目的观察艾灸对类风湿关节炎(RA)模型大鼠血清中白细胞介素IL-4、IL-6、IL-10、IL-1β、肿瘤坏死因子-α(TNF-α)及IL-23/IL-17炎症轴的变化的影响,探讨艾灸对RA模型大鼠相关炎性致损因子的干预作用。方法将40只SD大鼠随机分为对照组、模型组、艾灸组、药物组和香烟灸组,每组8只。除对照组外,其余4组采用风、寒、湿环境因素结合左后足跖皮下注射完全弗氏佐剂建立RA模型。对照组和模型组不作治疗;艾灸组予以左侧足三里艾灸,每日1次,每次20 min,共干预15 d;香烟灸组处理方法同艾灸组,用普通卷烟代替艾条;药物组予以雷公藤多苷片混悬液灌胃(每天0.8 mg/100 g),连续给药15 d。分别在造模前1天、造模后每3天,测量并计算左后足跖肿胀度以及关节炎指数;HE染色观察各组大鼠左后肢滑膜组织病理变化;采用酶联免疫吸附(ELISA)法测定各组大鼠血清IL-4、IL-6、IL-10、IL-1β、IL-23、IL-17和TNF-α的水平。结果关节肿胀度从治疗3 d至15 d,艾灸组与药物组均出现明显的下降趋势,艾灸组与药物组分别在治疗9 d至15 d、治疗12 d、15 d较模型组下降(P<0.05);关节炎指数从治疗3 d至15 d,艾灸组与药物组均在6 d至15 d较模型组下降(P<0.05);模型组出现明显的炎性浸润与滑膜增生,艾灸组较模型组明显改善,香烟灸组改善不明显;与对照组比较,模型组大鼠血清IL-6、IL-1β、TNF-α、IL-23、IL-17水平上升,IL-4、IL-10水平降低(均P<0.05);与模型组比较,艾灸组IL-6、IL-1β、TNF-α、IL-23、IL-17水平降低,IL-4、IL-10水平升高(P<0.05),药物组IL-6、IL-1β、IL-23、TNF-α水平降低,IL-4、IL-10水平升高(P<0.05),香烟灸组IL-6、IL-1β水平降低(P<0.05);与艾灸组比较,药物组IL-6、IL-4、IL-10的水平显著降低,IL-1β、IL-23水平升高(均P<0.05),香烟灸组IL-6、IL-1β、TNF-α、IL-23水平均显著升高,IL-4、IL-10均显著降低(P<0.05)。结论艾灸“足三里”可抑制IL-23Objective To observe the changes in serum interleukin(IL)-4,IL-6,IL-10,IL-1β,tumor necrosis factor(TNF)-αand the inflammatory axis IL-23/IL17 after the intervention with moxibustion in rheumatoid arthritis(RA)rat models,and to discuss the intervention effect of moxibustion on the associated inflammatory factors in RA rat models.Method Forty Sprague Dawley(SD)rats were randomized into a control group,a model group,a moxibustion group,a medication group and a cigarette moxibustion group,with 8 rats in each group.Except the control group,all the groups were put in a windy,cold and damp environment to receive subcutaneous injection of complete Freund’s adjuvant(CFA)at the left hind paw to develop RA models.The control group and model group didn’t receive any intervention;the moxibustion group was given moxibustion to the left Zusanli(ST36),once a day,20 min each time,for a total of 15 d;the cigarette moxibustion group received the same intervention as the moxibustion group but with ordinary cigarettes instead of moxa sticks;the medication group was intervened by intragastric administration of Tripterygium glycosides suspension at 0.8 mg/100 g each day,for 15 consecutive days.At 1 d before modeling and every 3 d after modeling,the swelling degree and arthritis index of the left hind paw were measured and calculated;HE staining was used to observe the pathological changes in the synovial tissues of rat’s left hind paw;the serum levels of IL-4,IL-6,IL-10,IL-1β,IL-23,IL-17 and TNF-αwere detected using enzyme-linked immunosorbent assay(ELISA).Result Regarding the joint swelling degree from treatment day 3 to day 15,it presented a notable decreasing tendency in the moxibustion group and medication group,and it was lower in the moxibustion group and the medication group than in the model group from treatment day 9 to day 15 and from treatment day 12 till day 15(P<0.05);regarding the arthritis index from treatment day 3 to day 15,it was lower in the moxibustion group and the medication group than in the model group
