穴位埋线联合二甲双胍对肥胖型多囊卵巢综合征患者糖脂代谢的影响

摘　　要：目的观察穴位埋线联合二甲双胍对肥胖型多囊卵巢综合征(PCOS)患者糖脂代谢的影响。方法肥胖型PCOS患者185例随机分为A组(二甲双胍组,60例)、B组(穴位埋线组,62例)、C组(穴位埋线联合二甲双胍组,63例)。比较治疗前后各组糖脂代谢指标[空腹葡萄糖(fasting plasma glucose,FPG)、空腹胰岛素(fasting insulin,FINS)、胰岛素抵抗指数(homeostatic model assessment of insulin resistance,HOMA-IR)、甘油三酯(triglyceride,TG)、总胆固醇(total cholesterol,TC)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDL-C)、脂质蓄积指数(lipid accumulation product,LAP)]的差异。结果3组治疗前各观察指标比较差异无统计学意义(P>0.05)。3组治疗后FPG、FINS、HOMA-IR、TG、TC、LDL-C、LAP均低于治疗前,HDL-C高于治疗前,差异均有统计学意义(P<0.05)。A组治疗后FPG、FINS、HOMA-IR、TC、LDL-C与B组比较差异无统计学意义(P>0.05),A组和B组治疗后FPG、FINS、HOMA-IR、TC、LDL-C均高于C组(P<0.05);A组治疗后HDL-C与B组比较差异无统计学意义(P>0.05),A组和B组治疗后HDL-C均低于C组(P<0.05);C组治疗后TG、LAP低于B组和A组(P<0.05),B组治疗后TG、LAP低于A组(P<0.05)。结论3种治疗方法均可改善肥胖型PCOS的糖脂代谢;穴位埋线联合二甲双胍疗法可以更加有效地改善肥胖型PCOS的糖脂代谢。Objective To observe the effect of acupoint thread-embedding plus Metformin on glucose and lipid metabolism in patients with obese polycystic ovary syndrome(PCOS).Method A total of 185 patients with obese PCOS were randomized into group A(Metformin group,60 cases),group B(acupoint thread-embedding group,62 cases)and group C(acupoint thread-embedding plus Metformin group,63 cases).The glucose and lipid metabolism indicators[fasting plasma glucose(FPG),fasting insulin(FINS),homeostatic model assessment of insulin resistance(HOMA-IR),triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C)and lipid accumulation product(LAP)]in each group were compared before and after treatment.Result There were no significant differences in the observed indicators across the three groups before treatment(P>0.05).After treatment,the FPG,FINS,HOMA-IR,TG,TC,LDL-C and LAP decreased and the HDL-C increased in the three groups,all with statistical significance(P<0.05).There were no significant differences in the FPG,FINS,HOMA-IR,TC and LDL-C between group A and group B after intervention(P>0.05),and the FPG,FINS,HOMA-IR,TC and LDL-C were significantly higher in groups A and B than in group C(P<0.05);there was no significant difference in the HDL-C between group A and group B after treatment(P>0.05),while the HDL-C was lower in groups A and B than in group C(P<0.05);the TG and LAP were lower in group C than in groups A and B after treatment(P<0.05);the TG and LAP were lower in group B than in group A(P<0.05).Conclusion The three treatment methods all can improve the glucose and lipid metabolism in obese PCOS;acupoint thread-embedding plus Metformin results in more significant improvement in glucose and lipid metabolism in obese PCOS.
