针灸应用于足月妊娠促宫颈成熟的疗效观察

摘　　要：目的观察针灸应用于足月妊娠促宫颈成熟的临床疗效。方法将180例足月需引产的产妇随机分为A组、B组和C组,每组60例。A组采用针灸疗法进行引产,B组采用Foley尿管水囊法进行引产,C组采用静脉滴注缩宫素混合液进行引产。观察各组第1、2产程时间和产后2 h出血量,并比较各组促宫颈成熟情况和剖宫产率。结果A组促宫颈成熟总有效率和剖宫产率分别为93.3%和10.0%,B组分别为75.0%和21.7%,C组分别为56.7%和46.7%。A组第1、2产程时间及产后2 h出血量、促宫颈成熟总有效率、剖宫产率与B组和C组比较差异均具有统计学意义(P<0.05)。B组第1、2产程时间及产后2 h出血量、促宫颈成熟总有效率、剖宫产率与C组比较差异均具有统计学意义(P<0.05)。结论针灸应用于足月妊娠促宫颈成熟疗效确切,能有效缩短产程,减少术后出血及剖宫产的发生。Objective To investigate the clinical promoting effect of acupuncture and moxibustion on cervical ripening in their application to term pregnancy.Method One hundred and eighty full-term pregnant women needed induced labor were randomized to groups A,B and C.Group A received acupuncture and moxibustion;group B,Foley urethral sac;group C,intravenous drips of oxytocin mixture,for induced labor.The lengths of the first and second stages of labor and the bleeding amount at two hrs after delivery were observed in various groups.Cervical ripening conditions and cesarean delivery rates were compared between various groups.Result The total efficacy rate in promoting cervical ripening and the cesarean delivery rate were 93.3%and 10.0%,respectively,in group A,75.0%and 21.7%,respectively,in group B and 56.7%and 46.7%,respectively,in group C.There were statistically significant differences in the lengths of the first and second stages of labor,the bleeding amount at two hrs after delivery,the total efficacy rate in promoting cervical ripening and the cesarean delivery rate between group A and group B or C(P<0.05)and between groups B and C(P<0.05).Conclusion Acupuncture and moxibustion have a definite promoting effect on cervical ripening in their application to term pregnancy.They can effectively shorten labor stage and reduce postoperative bleeding and the occurrence of cesarean delivery.
