摘　　要：目的:回顾性分析我院2015-2018年糖皮质激素类药物点评结果及存在的问题,对不合理使用情况进行统计分析,为药学干预提供方向,为临床合理用药提供参考。方法:将糖皮质激素类药物处方、医嘱从我院HIS及PASS系统导出,利用分层抽样法抽取处方1440份、医嘱960份,采用帕累托和回顾性分析方法分析我院糖皮质激素使用情况。结果:2400份糖皮质激素类药物处方医嘱中共有254份存在不合理现象,不合理率为10.58%,其中:药品剂型或给药途径不适宜89份,占35.04%;适应症不适宜56份,占22.05%;用法用量不适宜46份,占18.11%;遴选药品不适宜40份,占15.75%;临床诊断书写不全23份,占9.05%。结论:我院糖皮质激素类药物在近几年使用过程中存在不合理现象,需进一步强化糖皮质激素处方审核与专项点评工作,临床药师进行有效用药干预,加强临床使用管理监督,纳入医师考核,促进糖皮质激素类药物的临床合理使用。Objective:To retrospectively analyze the results and problems of glucocorticoids in our hospital from 2015 to 2018,and to conduct statistical analysis on irrational use,provide direction for pharmaceutical intervention,and provide reference for clinical rational drug use.Methods:The prescription and medical advice of glucocorticoids were derived from HIS and PASS system of our hospital,1440 prescriptions and 960 medical orders were extracted by stratified sampling,the use of glucocorticoids in our hospital was analyzed by pareto and retrospective analysis.Results:A total of 254 prescriptions of 2400 glucocorticoids were unreasonable,and the unreasonable rate was 10.58%.Among them,the drug dosage form or route of administration was not suitable for 89,accounting for 35.04%;the indications were not suitable for 56.Accounted for 22.05%;usage and dosage is not suitable for 46,accounting for 18.1%,40 drugs for injection,accounting for 15.75%,23 cases for clinical diagnosis,accounting for 9.05%.Conclusion:There are irrational phenomena in the use of glucocorticoids in our hospital in recent years.It is necessary to further strengthen the prescription review and special evaluation work for glucocorticoid drugs,effective medication intervention by clinical pharmacists,strengthen management and supervision in clinicalapplication,included in the assessment of doctors to promote the rational use of glucocorticoids.
